儿童外周血干细胞采集的安全性和有效性评估

【摘要】 目的评价应用血细胞分离机采集儿童外周血造血干细胞(PBSCs)的安全性和有效性。方法对本院2010年6月~2014年4月拟行自体PBSCs移植的50名患儿及17名健康儿童PBSCs供者,经G-CSF动员后、使用连续流动式COM.TEC血细胞分离机的auto-MNC程序采集PBSCs;对采集获得的CD34+细胞数做有效性评价,通过观察采集前后血液成分所受影响和采集过程中不良事件的发生情况做安全性评价。结果共采集67名儿童的PBSCs97例次,其中男性39人(56例次)、女性28人(41例次);中位年龄9岁(2~18)岁;中位体重28.5(11~70)kg,≥30kg者32人(45例次),<30 kg者35人(52例次)。采集成功率,1次占56.7%(38/67)、2次占41.8%(28/67)、3次占1.5%(1/67),平均采集(1.45±0.53)次。获得TNC细胞(221.82±260.94)×108个、CD34+细胞(115.55±167.09)×106个,CD34+细胞均≥2×106/kg;最常见的不良事件发生率依次为枸橼酸盐反应65.56%(59/90)、进液低压报警13.33%(12/90)、空气报警7.78%(7/90)、低血容量5.56%(5/90)、穿刺部位出血5.56%(5/90)、红细胞预灌注分离管路时漏血2.22%(2/90)。结论应用COM.TEC血细胞分离机采集儿童PBSCs是安全有效的。

【Abstract】 Objective To evaluate the safety and efficacy of the collection of peripheral blood stem cells(PBSC) by blood cell separator in children. Methods The subjects included children planning to receive autologous peripheral stem cell transplantation and healthy pediatric donors from June 2010 to April 2014. After mobilized by granulocyte-colony-stimulating factor(G-CSF),PBSCs were collected by COM. TEC blood cell separator with the Auto-MNC program. The number of CD34+ cells was employed as the indicator of collection efficacy. Safety was assessed by changes of blood cell counts before and after stem cell collection among different collection methods. Results Ninety-seven collections of peripheral stem cells were undertaken on 67 subjects(39 boys: 56 collections; 28 girls: 41 collections). Ages ranged from 2 to 18 years old with the median age of 9 years old. Body weights ranged from 11 ~ 70 kg(median: 28. 5 kg). Fifty-two collections were performed in35 children with body weight under 30 kg,while 45 collections undertaken in children whose body weight ≥ 30 kg. The average frequency was 1. 45 ± 0. 53. Among the 67 study subjects,collection of stem cells was successfully undertaken in one occasion,two occasions and three occasions in 38(56. 7%),28(41. 8%) and 1 children(1. 5%),respectively. The quantities of collected total nuclear cells(TNC) and CD34+cells were(221. 82 ± 260. 94) × 108and(115. 55 ± 167. 09) × 106,(CD34 + cells ≥ 2 ×106/kg). The most common adverse events were citrate reaction(65. 55%),alarm from low pressure of fluid input(13. 33%),air alert(7. 78%),low blood volume(5. 56%),oozing or hematoma at puncture sites(5. 56%),blood leakage(2. 22%). Conclusion The present study showed that collection of peripheral stem cells by COM. TEC blood cell separator is safe and efficacious.
