【摘要】 目的:探讨耳穴贴压对减轻四肢闭合性骨折患者术后伤口疼痛的效果。方法:首先进行选穴:根据患者四肢骨折的手术部位和术后诊断进行选取耳穴并进行贴压。取肾、肾上腺、神门、皮质下为主穴,如上肢骨折取肩、肘、腕穴为辅穴,下肢骨折取膝、踝、趾穴为辅穴;尔后进行耳穴贴压:在患者术后返回病房0.5h内,使用75%酒精棉球消毒耳廓待干,采用压痛法用探针以轻、慢而均匀的压力寻找压痛敏感点,确认贴压治疗的耳穴刺激点。用莱菔子耳穴贴在相应穴位上贴敷,并予适当按压,使耳廓有发热、胀痛感(即得气)。每个穴位以60～80次/min的频率进行按压,约5min/次左右,以患者感觉到酸、麻、胀为度,3h/次,夜间入眠后则适当延长按压间隔时间,双耳交替。对照组患者实施常规心理护理干预。遵医嘱肌注或口服曲马多镇痛。采用SPSS13.0软件进行统计学分析。结果:研究组患者术后4h、8h、12h和24h的疼痛程度较对照组减轻,使用曲马多止痛的病例减少,差异有统计学意义(P<0.05)。结论:四肢闭合性骨折术后的患者,采用耳穴贴压护理干预可有效地减轻术后疼痛,降低止痛药的使用。更多还原

【Abstract】 Objective: To investigate the clinical effect of applying auricular point sticking on traumatic patients with closed fracture duringpostoperative pain management. Methods: In the study group, we chose the points of auricular point sticking according to patients’ operative sites and postoperative diagnosis. The main points were Shen,Shenshangxian, Shenmen and Pizhixia. And the assistant points were Jian, Zhou and Wan among patients with upper limb fracture, as well Qi,Huai and Zhi among patients with lower limb fracture. Then, the treatment of auricular point sticking was used on them as follows. When patients got operations and were sent back to wards in half an hour, their ears would be sterilized with 75% concentrations of alcohol. We use probe to seek the sensitive points and confirmed them. Then, these related points were dressed with Laifuzi and pressed appropriately until their ears felt hot and painful. We pressed each point in both ears about 60 to 80 times per minute and continued five minutes. Patients got the treatment per three hours and may feel ache, swelling and numb. When they got sleep, we would prolong the interval of the treatment. And in the control group, patients got routine nursing care. According to the pain classification, patients in both groups might get tramadol in oral or muscle by doctors’ prescriptions if necessary. Results: The pain managements were better in the study group than in the control group during 4hours, 8hours, 12 hours and 24 hours after operations, and use less doses of tramadol, and could be adopted easily by patients (P<0.05). Conclusion: The treatment of auricular point sticking could be used on traumatic patients with closed fracture during postoperative pain management and decrease the dose of painkillers.更多还原
