2009—2011年西安市某医院铜绿假单胞菌耐药性分析

【摘要】 目的分析兵器工业五二一医院2009年4月—2011年12月铜绿假单胞菌耐药性,为抗菌药物的合理使用及管理提供参考。方法采用常规方法对医院各类标本进行分离培养,用HX-21细菌鉴定药敏分析仪,对细菌进行鉴定和抗菌药物敏感性测定,数据通过HX-21分析软件进行对比统计分析。结果铜绿假单胞菌主要来自痰标本,分布以呼吸科、重症监护室为主;2009—2011年铜绿假单胞菌对非限制级抗菌药物的耐药性呈上升趋势,对头孢3代以上、β内酰胺酶抑制剂、碳青酶烯类抗菌药物的耐药性呈下降趋势。特别是2011年开展抗菌药物临床使用专项整治活动以来,铜绿假单胞菌对哌拉西林舒巴坦、头孢哌酮舒巴坦、亚胺培南耐药性大幅降低,分别为3.45%,6.06%、11.2%。结论头孢哌酮-舒巴坦、哌拉西林-舒巴坦、亚胺培南对铜绿假单胞菌有较好的体外活性,为及时有效地控制铜绿假单胞菌的感染,应根据药敏结果合理选用抗菌药物。同时多部门、多学科共同参与的抗菌药物临床使用管理可以有效控制细菌耐药性。

【Abstract】 [Objective]To analyze the drug resistance of Pseudomonas aeruginosa in No.521 Hospital of Weapons Industry from April 2009-December 2011,and provide reference for rational use of antimicrobial drugs.[Methods]Various specimens collected from hospital were isolated and cultured via conventional method.HX-21 bacterial sensitivity analyzer was adopted to identify the bacterial and determine antimicrobial susceptibility.The data was statistically analyzed through HX-21 analysis software.[Results] Pseudomonas aeruginosa was mainly isolated from sputum samples,and mainly distributed in respiratory department and ICU.The resistance of Pseudomonas aeruginosa to non-restricted antimicrobial agents increased from 2009-2011,while that to tertiary-generation cephalosporin or above,β-lactamase inhibitors,carbapenem antibacterial drugs was decreased.Since the special rectification activities of clinical use of antimicrobial drugs in 2011,the resistance of Pseudomonas aeruginosa to piperacillin sulbactam,cefoperazone sulbactam,imipenem was significantly decreased by 3.45%,6.06%,11.2%,respectively.[Conclusion]Cefoperazone-sulbactam,piperacillin-sulbactam,and imipenem have a better in vitro activity for Pseudomonas aeruginosa.In order to control the Pseudomonas aeruginosa infection,rational use of antimicrobial drugs should be based on susceptibility results.Multisectoral and multidisciplinary management of antimicrobial clinical use can effectively control bacterial resistance.
