左室心肌致密化不全心肌病的临床特征和超声心动图表现
Clinical features and echocardiographic characteristics in patients with left ventricular noncompaction cardiomopaction
【摘要】 目的分析左室心肌致密化不全心肌病(LVNC)的临床特征和超声心动图表现。方法回顾性分析南华大学附属第一医院2008年10月~2012年10月48例LVNC患者的临床特征,采用Philips IE33和Philips Sonos 5500超声诊断仪对患者进行多切面扫查,依据17节段法分析受累节段范围、程度及左室心功能。结果 48例患者,35例(72.9%)患者左室射血分数(EF)<50%,平均EF值(32.0±7.3)%,13例(27.1%)患者左室EF≥50%,平均EF值(64.8±6.5)%。36例(75%)患者伴有二尖瓣关闭不全,2例患者伴有左室乳头肌致密化不全。4例(8.3%)患者合并各种先天性心脏病(Fallot四联症、主动脉瓣二瓣化畸形、室间隔缺损等)。48例患者受累节段296个,主要累及左室心尖部、心尖段各室壁、中间段后侧壁及下壁,其中34例患者心尖部受累。超声心动图受累节段均见丰富肌小梁回声及深陷其间的隐窝,CDFI隐窝内可见与心腔相通的低速血流信号,收缩末期非致密心肌/致密心肌厚度(NC/C)比值≥2,本研究NC/C为(3.1±0.8)(2~6)。结论超声心动图检查能够全面评价LVNC房室结构及心功能,有助于临床上明确心力衰竭病因并协助治疗,是诊断及筛查此病的最佳方法。
【Abstract】 【Objective】To investigate the clinical features and echocardiographic characteristics of the left ventricular noncompaction cardiomopaction.【Methods】The clinical feature were analyzed in 48 patients with LVNC from October 2008 to October 2012 in this retrospective study.The patients were performed multiple views by using ultrasonic diagnostic instrument,which included the type of Philips IE33 and Philips Sonos 5500,on the basis of the 17 segment method analysis involved segment range,degree and left ventricular cardiac function.【Results】Among those patients,there were 35 cases of patients(72.9%) with left ventricular ejection fraction(EF)<50%,average EF value(32.0±7.3)%,13 cases of patients(27.1%) with left ventricular EF ≥50%,and the average value was(64.8± 6.5)%.There were 36 cases of patients(75%) who had mitral valve RAL regurgitation,and 2 of them suffer from the left ventricular noncompaction.4 cases(8.3%) complicated with congenital heart disease(tetralogy of Fallot,aortic valve two dischange deformity,ventricular septal defect,etc).There were 296 affected segments that were found in 48 patients.The left ventricular apex department,apex section of the chamber wall,intermediate section after the wall and wall were mainly involved,among them there were 34 cases of patients,who involved the department of apex.Multiple prominent trabeculations and deep recesses were found in affected sections by the CDFI of echocar diography,and found that there were blood flow information signal in low velocity,between the deep recesses and the heart chamber.The myocardial thickness ratio of between noncompaction myocardial and compaction was≥2 in endsystolic,and the NC/C was(3.1±0.8)(2~6) in this study.【Conclusions】The structures of the heart in LVNC and the ventricular function can be evaluated by echocardiography,and will be helpful to find the etiiology of heart failure,so as to help the treatment.The echocardiography is considered the best diagnostic modality for this disease.
