北京地区部分人群游离甲状腺激素与吸烟及代谢指标的关系分析

【摘要】 目的研究甲状腺激素与吸烟及其他指标如年龄、性别、空腹胰岛素、血糖、甘油三酯及胆固醇等的关系。方法共选择237例受试者,其中118例女性,119例男性,年龄37岁~63岁。测定受试者的空腹促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4)、胰岛素(FINS)、血糖(FBG)及血脂水平,并进行统计学分析。结果与不吸烟者相比,吸烟者体内FT3(P<0.01)及TG(P<0.05)水平要高;而血糖(P<0.01)及HDL-C(P<0.001)浓度要低。FT3水平与BMI(p=0.001)、FINS(P<0.001)及TG(P<0.01)水平呈直接正相关,且与HDL-C(P<0.01)水平负相关。多因素线性回归分析中以FT3作为因变量,以吸烟、性别、TSH、FINS、TG及HDL-C等因素为自变量,结果发现FT3与吸烟习惯(P<0.05)、FINS(P<0.001)及男性(P<0.001)均呈显著正相关。结论血清FT3水平升高与吸烟有关,且独立于性别及FINS等代谢参数。

【Abstract】 Objective To study the relationship between thyroid hormones and smoking,as well as several other parameters like age,gender,fasting insulin,glucose,triglycerides and cholesterol. Methods A total of 237 subjects between 37 ~ 63 years old,118 women and 119 men,were involved to determine the TSH,FT3,FT4,FINS,FBG and blood lipid levels,and the results were analyzed by statistics. Results Smokers showed higher FT3( P < 0. 01) and triglyceride( P < 0. 05) levels and lower FBG( P < 0. 01) and HDL- cholesterol( P < 0. 001) concentrations than non- smoking subjects. FT3 levels were positively associated with BMI( P = 0. 001),FINS( P < 0. 001),and triglyceride( P < 0. 01) levels and negatively correlated with HDL- cholesterol level( P < 0. 01). When a multiple regression analysis performed with FT3 level as the dependent variable,while smoking,gender,TSH,FINS,triglyceride,and HDL- cholesterol serum concentrations as independent variables,FT3 levels maintained an independent positive association with smoking( P < 0. 05),FINS( P < 0. 001) and male( P < 0. 001). Conclusion Smoking may increase FT3 level independently of gender,insulin and metabolic parameters.
