尿酶与微量蛋白联合检测在早期肾损伤诊断中的应用价值

【摘要】 目的探讨尿N-乙酰-β-氨基葡萄糖苷酶(NAG)联合尿微量蛋白检测在糖尿病和高血压病早期肾损伤的诊断及临床价值。方法收集152例糖尿病和122例高血压病患者及60例正常对照者,对其尿β2微球蛋白(β2-MG)、免疫球蛋白G(IgG)、微量白蛋白(mALB)、α1微球蛋白(α1-MG)及NAG水平进行检测并分析。结果糖尿病与高血压病尿蛋白阳性组4种微量蛋白及NAG水平明显高于尿蛋白阴性组与对照组(P<0.001),尿蛋白阴性组4种微量蛋白及NAG水平高于对照组(P<0.05)。糖尿病与高血压病尿蛋白阴性性组NAG联合4种尿微量蛋白检测阳性率明显高于各单项检测阳性率(P<0.001),与4种微量蛋白联合检测阳性率比较亦升高(P<0.05)。结论尿N-乙酰-β-氨基葡萄糖苷酶(NAG)与尿微量蛋白联合检测对早期肾脏损害的诊断具有重要临床价值。

【Abstract】 Objective To evaluate the clinical significance of joint detection of urinary N- acetyl- β- D- glucosaminidase( NAG) and microprotein for diagnosis of early renal damage in patients with diabetes or hypertension. Methods The levels of urinary β2- microglobuin,α1- microglobuin,microalbumin,IgG and NAG were measured and analyzed in 152 patients with diabetes,122 patients with hypertension and 60 healthy subjects. Results In patients with diabetes or hypertension,the levels of NAG and four kinds of microprotein were significantly higher in urine protein positive group compared with urine protein negative group and control group( P < 0. 001),and higher in urine protein negative group compared with control group( P < 0. 05).In urine protein negative group of diabetes or hypertension,the positive rate of joint detection of NAG and four kinds of microprotein was significantly higher than that of any single detection( P < 0. 001) and joint detection of four kinds of microprotein( P< 0. 05). Conclusion The joint detection of urinary NAG and microprotein had important clinical value in the diagnosis of early renal damage.
