ANA谱和ds-DNA检测在系统性红斑狼疮诊断中的临床意义

【摘要】 目的:探讨抗核抗体谱(ANA谱)检测和不同方法双链DNA(ds-DNA)检测在系统性红斑狼疮(SLE)诊断中的临床意义。方法:收集162例SLE患者和50例健康对照者的血清,用间接免疫荧光法(I-IF)检测抗核抗体(ANA),用酶联免疫吸附法(ELISA)、欧蒙印迹法和间接免疫荧光法检测抗ds-DNA抗体,用欧蒙印迹法测定抗核抗体谱。结果:SLE患者ANA阳性率为90.3%;ELISA、欧蒙印迹法、IIF检测抗ds-DNA抗体的阳性率分别是25.3%、37.6%、35.8%,抗ds-DNA抗体总体阳性率为49.0%;抗核抗体谱中抗U1-nRNP抗体、抗Sm抗体、抗SS-A抗体、抗SS-B抗体、抗核小体抗体、抗组蛋白抗体、抗核糖体P蛋白检测的阳性率分别是25.3%、9.9%、63.0%、7.4%、20.4%、22.2%、24.1%,均明显高于健康对照组(P<0.05)。结论:ANA谱和ds-DNA检测提高了SLE的检测敏感度和诊断率,可减少SLE的误诊和漏诊情况,对SLE的诊断和治疗有重要的临床意义。

【Abstract】 Objective:To investigate clinical significance of autoantibody and ds-DNA in diagnosis of systemic lupus erythematosus(SLE).Methods:Sera from one hundred and sixty two patients with SLE and fifty healthy controls were collected.IIF was used to detect ANA.ELISA,Southern blot and IIF(indirect immunofluorescence) were applied to assay ds-DNA.Southern blot was employed to measure ANA profile.Results:The sensitivity of ANA and ds-DNA in SLE were 90.3%,49.0% respectively.The comprehensive sensitivity of ds-DNA in SLE was 49.0%.The positive rates of ELISA,Southern blot and IIF in ds-DNA antibody detection in SLE were 25.3%,37.6% and 35.8% respectively.anti-U1RNP antibody,anti Sm antibody,anti SSA annibody,anti SSB antibody,anti-Nucleosome antibody,anti-Histone antibody,anti-Ribosomal P-protein autoantibody in SLE were 25.3%、9.9%、63.0%、7.4%、20.4%、22.2%、24.1% respectively.The antibodies mentioned above in SLE were higher than those of healthy controls(P<0.05).Conclusion:The determination of autoantibodies and dsDNA may improve the sensitivity and diagnostic efficiency,so it play an important role in the diagnosis of SLE.
