【摘要】 目的:观察选择性背根神经节脉冲射频术联合普瑞巴林治疗带状疱疹后神经痛的临床疗效及安全性。方法:100例带状疱疹后神经痛患者,采用数字表法随机均分为两组,对照组(n=50):口服普瑞巴林300 mg/d治疗;观察组(n=50):在口服普瑞巴林300 mg/d治疗的基础上联合选择性背根神经节脉冲射频术治疗。观察治疗前及治疗后不同时期(术后1月、术后2月和术后3月)疼痛程度和睡眠质量的变化。采用简化McGill评分(short-form of McGill pain questionnaire,SF-MPQ)及睡眠质量综合评定治疗效果,并记录并发症和不良反应的发生率。结果:两组治疗后各时期SFMPQ评分均较治疗前明显降低,睡眠质量评分均较治疗前明显增加(P<0.01);观察组治疗后各时期SF-MPQ评分较对照组同期明显降低,睡眠质量评分较对照组同期明显升高(P<0.01)。观察组在治疗后12 h内未出现颈部血肿和气胸。两组患者在口服普瑞巴林后出现程度不等的不良反应,患者均可耐受。结论:选择性背根神经节脉冲射频术联合普瑞巴林治疗带状疱疹后神经痛效果好,可迅速缓解患者疼痛症状、改善患者睡眠质量且安全性良好。更多还原

【Abstract】 Objective: To investigate the efficacy and safety of selective dorsal root ganglion pulsed radiofrequency combined with pregabalin in treatment of postherpetic neuralgia(PHN).Methods: One hundred patients with PHN were randomly divided into two groups of 50 each according to digital table.The control group was given with pregabalin 300 mg/d;the observation group was given with pregabalin 300 mg/d combined with selective percutaneous dorsal root ganglion radiofrequency.Changes in pain degree and sleep quality were observed before treatment and at different time points after treatment(the 1st month,2nd month and 3rd month).The short-form McGill pain questionnaire(SF-MPQ) and sleep quality scores were used for evaluation of treatment effcacy,and frequencies of complications and adverse reactions were recorded.Results: The SF-MPQ of two groups decreased signifcantly at different time points after treatment compared with before treatment(P < 0.01),and the sleep quality score was increased signifcantly at different time points after treatment compared with before treatment(P < 0.01).The SF-MPQ of observation group decreased signifcantly after treatment compared with control group(P < 0.01),the sleep quality of observation group increased signifcantly after treatment compared with control group(P < 0.01).No neck hematoma and pneumothorax developed within 12 hours after treatment in observation group.The patients in two groups developed variable degrees of adverse reactions after oral pregabalin treatment,but were well tolerated.Conclusion: Selective percutaneous dorsal root ganglion radiofrequency combined with pregabalin can rapidly alleviate postherpetic neuralgia pain,enhance patients’ quality of sleep,and has good safety,which was effective in treatment of PHN.更多还原
