3所医院铜绿假单胞菌中ESBLs基因的检测
Detection of genes of extended-spectrum β-lactamases produced by Pseudomonas aeruginosa in three hospitals
【摘要】 目的了解昆明3所医院临床分离的铜绿假单胞菌中超广谱β-内酰胺酶(ESBLs)基因的分布情况。方法收集昆明3所医院临床标本中分离的铜绿假单胞菌110株,用VITEK32型全自动细菌鉴定仪进行细菌鉴定,用聚合酶链反应法对其进行基因检测,靶基因为TEM、SHV、OXA、PER、VEB、GES、CTX-M-1共7种ESBLs基因,对检测的阳性产物进行测序,并对照GenBank确定其基因型。结果 110株PA中7种主要的ESBLs基因型(PER、VEB、GES、TEM、SHV、OXA、CTX-M)均有检出,其中TEM型检出率占77.3%。结论 110株PA中ESBLs基因型分布较广,TEM是其中流行的主要基因型,各医院应规范临床用药,并加强联合动态监测。
【Abstract】 Objective To investigate the genes of extended-spectrum β-lactamases(ESBLs) produced by clinical isolated Pseudomonas aeruginosa(PA) from 3 hospitals in Kunming.Methods 110 clinical isolates of Pseudomonas aeruginosa were collected from 3 hospitals in Kunming and identified with VITEK-32 automated bacteria identification instrument.And polymerase chain reaction(PCR) analysis was used to detect 7 kinds of ESBLs genes including TEM,SHV,OXA,PER,VEB,GES,CTX-M-1.DNA sequencing was used to detect positive production of PCR with GenBank as control.Results 7 kinds of ESBLs genes(PER,VEB,GES,TEM,SHV,OXA,CTX-M)were all detected in 110 clinical isolates of Pseudomonas aeruginosa.Among the 7 kinds of ESBLs genes,the detection rate of TEM was 77.3%.Conclusion The distribution of ESBLs genotypes was varied in 110 clinical isolates of Pseudomonas aeruginosa.TEM was a main genotype among prevalent ESBLs genotypes.For local hospitals,attention must be paid to use antibiotics reasonablely and strengthen laboratories associated surveillance dynamically.
