【摘要】 目的检测原发性肝癌患者血清中磷脂酰肌醇蛋白聚糖3(glypican-3,GPC3)和甲胎蛋白(AFP)水平,比较二者对肝癌的诊断价值,并探讨二者的相关性。方法对120例肝癌、32例肝硬化病人及25例正常人血清标本,通过ELISA法检测标本GPC3值,电化学发光法检测标本AFP值并对结果进行分析。结果 (1)肝癌组GPC3值为22.014±36.930ng/ml,与正常对照组及肝硬化组之间差异存在显著性(P<0.01)。(2)GPC3对肝癌诊断的敏感度及特异度分别为85.83%和50.00%;AFP对肝癌诊断的敏感度及特异度分别为83.33%和62.50%,GPC3与AFP之间差异没有显著性(P>0.05);二者联合检测敏感度及特异度分别为95.83%和46.88%,敏感度有所提高。(3)肝癌组GPC3与AFP之间无相关性(P>0.05)。结论 GPC3作为一种新的肿瘤标志物对肝癌诊断的敏感性较高,GPC3与AFP联合诊断更能提高肝癌的检出率,但二者之间并无相关性。更多还原

【Abstract】 Objective To compare the diagnostic value between phosphatidylinositol proteoglycan 3(Glypican-3,GPC3) and alpha-fetoprotein(AFP) levels of serum for patients with primary hepatocellular carcinoma and discuss their relevance.Methods Serum GPC3 and AFP were measured by ELISA and the electrochemical luminescence method in 120 patients with primary hepatocellular carcinoma,32 patients with hepatic cirrhosis and 25 normal people.Results(1)The value of GPC3 in hepatocellular carcinoma group(22.014± 36.930 ng/ml)was significantly higher than control group and hepatic cirrhosis group(P<0.01).(2)Sensitivity of GPC3 for patients with primary hepatocellular carcinoma was 85.83%,and specificity was 50.00%.Sensitivity of AFP for patients with primary hepatocellular carcinoma was 83.33%,and specificity was 62.50%,Sensitivity of GPC3 and AFP was 95.83%,and specificity was 46.88%.The sensitivity was higher.(3) The GPC3 and AFP for patients with primary hepatocellular carcinoma were not significant differences(p >0.05).Conclusion As a new tumor marker in primary hepatocellular carcinoma,GPC3 has a higher sensitivity,GPC3 in combination with AFP can improve the detection of HCC,but no relativity exists between them.更多还原
