人工种植牙龈下中间普氏菌1年内数量变化研究
Study on the changes of subgingival Prevotella intermedia of dental implant within one year
【摘要】 目的研究人工种植牙龈下中间普氏菌(Prevotella intermedia,Pi)1年内的数量变化情况,为种植体的定期维护提供理论基础。方法于2010年6月到2011年6月,对在山西医科大学第一医院口腔科门诊已完成固定义齿修复的38例患者的46颗人工种植牙进行为期1年的复诊检查,记录修复后1、3、6、12个月4个时段,所有种植牙的菌斑指数(PLI)、牙龈指数(GI)、牙龈出血指数(GBI)、探诊深度(PD)以及X线骨吸收情况,并采用Pi的选择性培养基对龈下菌斑标本进行分离培养,计算每毫升标本原液的菌落形成单位(CFU/mL)。结果随时间延长,种植牙周Pi检出量不断增加,1个月到3个月时增加的趋势最为明显,到6个月左右趋于稳定,其统计值在1个月和3个月、3个月和6个月间比较,差异有统计学意义(P<0.05),但在6个月和12个月间比较,差异无统计学意义(P>0.05)。PLI、GI和X线骨吸收值在修复后1、3、6、12个月比较,差异均有统计学意义(P<0.05)。GBI和PD仅在1个月和3个月间比较,差异有统计学意义(P<0.05),其余各时段间比较,差异均无统计学意义(P>0.05)。结论在临床工作中,应注意加强修复后3个月时种植患者的口腔维护,防止种植体周围炎的发生,提高种植成功率。
【Abstract】 Objective To study the changes of Prevotella intermedia Pi of the implants within one year. Methods Forty-six implants of 38 patients were collected. In the four stages of one month , 3 months , 6 months and 12 months after restoration , evaluate their Plaque Index ( PLI ), Gingival Index ( GI ), Gingival Bleeding Index ( GBI ), Probing Depth ( PD ) and X-ray bone absorption. Subgingival plaque samples were cultured separately by the selective medium of Pi . Results As time went on , the value of Pi was increasing. From the 1st to the 3rd month , the value increased most obviously and by the 6th month it became stable. The value of Pi was statistically different between the 1st and the 3rd month , the 3rd month and the 6th month , and showed no statistical difference between the 6th and 12th month. PLI , GI and X-ray bone absorption were statistically different among 1st , 3rd , 6th and 12th month (P < 0.05 ); the GBI and PD was significantly different between the 1st and the 3rd month (P< 0.05 ) . Other tested indexes showed no statistic difference (P > 0.05 ) . Conclusion Patients with implants should be guided to do cleansing and maintenance in oral cavity after 3 months of restoration , so as to prevent peri-implantitis and increase success rate of implants.
