44株甘肃疑似非结核分枝杆菌临床分离株的菌种鉴定

【摘要】 目的对甘肃44株疑似非结核分枝杆菌临床分离株进行菌种鉴定。方法收集2010年甘肃省兰州市胸科医院从结核病患者痰标本中分离的、抗酸染色阳性和改良罗氏培养基,TCH和PNB鉴别培养基培养鉴定为非结核分枝杆菌(non-tuberculosis Mycobacteria,NTM),采用rpoB-PRA和PCR产物DNA测序等分子生物学方法进行细菌种的鉴定。结果共对44株疑似非结核分枝杆菌临床分离株进行了鉴定。经rpoB-PRA、rpoB和hsp65基因DNA测序分析,其中非结核分枝杆菌13株(29.55%),包括龟分枝杆菌(M.chelonae)5株,戈登分枝杆菌(M.gordonae)4株,胞内分枝杆菌(M.intracellulare)2株,赛特分枝杆菌(M.setense)1株,耻垢分枝杆菌(M.smegmatis)1株;诺卡菌(Nocardia)31株(70.45%),其中鼻疽诺卡菌(N.farcinica)30株,卡尔涅亚诺卡菌(N.carnea)1株。结论抗酸染色阳性、PNB/TCH鉴定为非结核分枝杆菌临床分离株中包含不同种类的非结核分枝杆菌和诺卡菌,应开展进一步菌种鉴定,以便于临床正确诊断和合理治疗。

【Abstract】 The suspicious clinical isolates of non-tuberculosis mycobacteria(NTM) were identified in Gansu,China in this study.Suspicious NTM isolates were collected from clinical samples(acid-fast bacilli) in the Lanzhou Pulmonary Hospital,Gansu Province,China,which were preliminary identified with L-J and PNB/TCH differential media.Results indicated that 44 suspicious NTM isolates were identified.Using PCR-restriction fragment length polymorphism analysis(PRA) of the rpoB gene(rpoB-PRA) and DNA sequencing based on hsp65 and rpoB gene,13(29.55%) NTM contained 5 M.chelonae,4 M.gordonae,2 M.intracellulare,1 M.setense and 1 M.smegmatis,31(70.45%) Nocardia contained 30 N.farcinica and 1 N.carnea.In conclusion,NTM that were identified with Ziehl-Neelsen acid-fast staining and L-J and PNB/TCH differential media contained different NTM species and Nocardia genus.It’s essential to identify more accurate species for clinical diagnosis and treatment.
