鲍曼不动杆菌氨基糖苷类修饰酶基因研究
Study on aminoglycoside-modifying enzyme genes in Acinetobacter baumannii
【摘要】 目的:对临床分离鲍曼不动杆菌耐药性及氨基糖苷类修饰酶基因进行研究,以指导临床合理应用抗生素。方法:K-B琼脂纸片扩散法进行药敏试验,PCR法检测8种氨基糖苷类修饰酶基因,并对PCR产物进行测序。结果:70株ABA对13种药物的耐药率均在65%以上,仅对头孢哌酮/舒巴坦、亚胺培南、美洛培南的敏感率较高。氨基糖苷类修饰酶基因总检出率为64.29%(45/70),其中aac(6’)-Ⅰ检出率最高为41.40%,其次为基因aac(3)-Ⅱ,aac(3)-Ⅰ和ant(3〞)-Ⅰ,检出率分别为34.29%,31.435%和25.71%;未检出aac(6’)-Ⅱ,ant(2〞)-Ⅰ,aph(3’)-Ⅰ及aph(3’)-Ⅱ基因。结论:氨基糖苷类修饰酶基因aac(6’)-Ⅰ、aac(3)-Ⅱ、aac(3)-Ⅰ及ant(3〞)-Ⅰ为本地区鲍曼不动杆菌对氨基糖苷类药物耐药的主要原因。
【Abstract】 Objective:To investigate antibiotic resistance and aminoglycoside-modifying enzyme genotype in A cinetobacter baumannii,so as to guide the rational use of antibiotics in clinical.Methods:K-B method was used to do drug susceptibility test;PCR was applied to detect 8 types of aminoglycoside-modifying enzyme genes,and the positive products of genes were sequenced.Results: Among the 70 isolates,the resistant rates to 13 of 18 drugs were more than 65%,while the sensitive rates to Cefoperazone/sulbactam,imipenem,meropenem were higher.In the 70 strains of Acinetobacter baumannii,the total positive rate of AMEs genes was 64.29%(45/70),the positive rates of aac(6′)-Ⅰ,aac(3)-Ⅱ,aac(3)-Ⅰ,and ant(3〞)-Ⅰwere 41.40%,34.29%,31.435% and 25.71%,respectively;all strains were not detected aac(6′)-Ⅱ,ant(2〞)-Ⅰ,aph(3′)-Ⅰ,aph(3′)-Ⅱgenes.Conclusions: The gene existence of aac(3)-Ⅰ,aac(3)-Ⅱ,aac(6′)-Ⅰ,ant(3〞)-Ⅰare the main resistance mechanism of A.baumannii to aminoglycoside antibiotics in this region.
