去甲基化药物在异基因造血干细胞移植中的临床应用进展
Clinical application of hypomethylating agents in allogeneic hematopoietic stem cell transplantation
【摘要】 肿瘤表观遗传学的研究发现DNA甲基化异常在血液肿瘤的发生和转化中起着重要作用,基础和临床研究发现去甲基化药物不仅具有抑制甲基化转化酶的作用,有效治疗骨髓增生异常综合征和急性髓细胞白血病,而且具有免疫调节作用,因此在异基因造血干细胞移植中也显示出它特有的临床价值:增强移植物抗白血病效应而不增加移植物抗宿主病的发生。本文将综述甲基化药物在异基因造血干细胞移植这一领域的应用进展。
【Abstract】 Investigations on cancer epigenetics have found that DNA methylation played an important role during pathogenesis and transformation of hematopoietic malignancies. The DNA methyltransferases inhibitors azaciditine and decitabine have obtained encouraging efficacy in treatment of myelodyspalstic syndrome and acute myeloid leukemia. Hypomethylating agents not only exert anti-tumor effect, but also have immunomodulatory effects that extend their use in the allogeneic hematopoietic stem cell transplantation(allo-HSCT) scenario. It has been hypothesized that hypomethylating agents may augment the graft-versus-leukemia effect without increasing graft-versus-host disease after allo-HSCT. Here we present a review of advancement of hypomethylating agents in allo-HSCT.
