肝癌患者外周血中T淋巴细胞表面标志潜态相关性肽链的表达特点及意义

【摘要】 目的研究肝癌患者外周血中T淋巴细胞表面标志潜态相关性肽链(LAP)的表达特点以及肝癌根治性切除术对LAP表达的影响,探讨其在肝癌发生发展中的作用。方法抽取北京大学深圳医院2011年10月到2012年12月期间30例肝癌患者的外周静脉血与30名健康志愿者的外周静脉血,采用流式细胞术检测LAP+CD4+T细胞在肝癌患者和正常人外周血单个核细胞中的比例,并检测LAP+CD4+T细胞在肝癌患者术前和术后的外周血单个核细胞中的比例。结果 LAP+CD4+T细胞在肝癌患者外周血中的表达水平明显高于正常组(1.84±0.85 vs.0.73±0.39,P=0.006),差异有统计学意义。肝癌患者外周血中LAP+CD4+T细胞术后组比术前组显著下降(1.07±0.35 vs.1.84±0.85,P=0.001),差异有统计学意义。肝癌术后组和正常组相比,LAP+CD4+T细胞略微有所升高,但差异无统计学意义(1.07±0.35 vs.0.73±0.39,P=0.209)。结论 LAP+CD4+T细胞在肝癌患者外周血中的表达比例明显上升,肝癌根治性切除术后患者外周血中的LAP+CD4+T细胞的比例较术前显著下降并接近正常水平,提示肝癌患者外周血中高水平的具有抑制效应的LAP+CD4+T细胞可能是肝癌局部微环境中负性调节机制的重要组成,在肝癌免疫逃逸中发挥积极作用。

【Abstract】 Objective To study the expression characteristic of T lymphocytes surface marker LAP in peripheral blood mononuclear cells(PBMC) of human hepatocellular carcinoma(HCC) patients and to explore the change of LAP level in PBMC of HCC patients after radical hepatectomy. Methods Thirty HCC patients hospitalized during October 2011 to December 2012 were enrolled in the study. The control group consisted of thirty healthy persons. The proportions of LAP+CD4+T cells in PBMC of HCC patients, 3 days pre-operatively and 10 days post-operatively, were detected by flow cytometry(FCM), compared with those of healthy persons. Results The proportions of LAP+CD4+T cells in PBMC of HCC patients were significantly higher than those of healthy persons(1.84±0.85 vs 0.73±0.39, P=0.006<0.05). The proportions of LAP+CD4+T cells in PBMC of HCC patients post-operatively were manifestly reduced with those pre-operatively(1.07±0.35 vs. 1.84±0.85, P= 0.001<0.05). Conclusions The expression proportions of LAP+CD4+T cells were markedly increased in PBMC of HCC patients and significantly decreased to near the normal level after radical hepatectomy. Our results demonstrate that higher level of LAP+CD4+T cells,with suppressive effects, in PBMC of HCC patients may be an important component of negative regulation mechanism in the local tumor microenvironment and will accelerates the immune escape in the development of HCC.
