CD105标记微血管密度及HIF-1α在乳腺癌及癌前病变中的表达及意义
Expressions of HIF-1α, CD105-MVD and clinical significance in breast carcinoma and precancerous lesions
【摘要】 目的研究CD105标记的微血管密度(MVD)及HIF-1α在乳腺癌和癌前驱病变中的表达及其与生物学行为的关系。方法应用免疫组化UltraSensitiveTMS-P法检测57例乳腺非典型导管增生(ADH)、89例乳腺导管原位癌(DCIS)和128例乳腺浸润性导管癌非特殊类型(IDC-NST)组织中HIF-1α、CD105-MVD的表达,并与其临床病理因素对照,以53例乳腺普通导管增生(UDH)标本作对照。结果 (1)HIF-1α在NST中的阳性表达为68.75%,明显高于DCIS(43.8%)、ADH(31.6%)、UDH(9.4%),差异均有统计学意义(χ2=13.442,22.272,52.789,P<0.01);DCIS与ADH比较差异无统计学意义(χ2=2.1878,P=0.139)。(2)NST中CD105-MVD(31.691±8.621)与DCIS(27.633±6.879)、ADH(21.436±5.112)和UDH(10.038±3.976)比较,IDC-NST与DCIS(t=3.696)、ADH(t=10.056)与UDH(t=22.105)比较,DCIS与ADH比较(t=4.873),差异有统计学意义(P<0.05);病变周围区域CD105-MVD明显高于病变中央区域(P<0.01)。(3)在IDC-NST中,HIF-1α、CD105-MVD的表达与患者年龄、肿瘤大小无关(P>0.05),而与激素ER(PR)表达、组织学分级、淋巴结及远处转移相关(P<0.05)。(4)在IDC-NST中,HIF-1α阳性组CD105-MVD表达(35.120±5.541)高于阴性组(22.040±2.082),差异有统计学意义(P<0.01)。结论 HIF-1α和CD105-MVD高表达与乳腺癌发生、侵袭转移行为有关,HIF-1α可能发生在癌变早期阶段(UDH和DCIS),HIF-1α和CD105-MVD联合检测有望作为判断乳腺癌预后的指标及分子靶向治疗的靶点。
【Abstract】 Objective To study the expression of HIF-1α, CD105 and microvascular density(CD105-MVD)in breast carcinoma and precancerous lesions and investigate their associations with invasion and metastasis of breast carcinoma. Methods Immunhistochemical UltraSensitiveTMS-P method was employed to detect the expression of HIF-1α, CD105-MVD in 57 cases of atypical ductal hyperplasia(ADH), 89 cases of ductal carcinoma in situ(DCIS), and 128 cases of invasive ductal carcinomas of no special type(IDC-NST) of the breast, 53 cases of usual ductal hyperplasia(UDH) breast tissues were selected as a control group. Results(1) The positive rates of HIF-1α were 68.75%, which were higher than that in DCIS(43.8%), ADH(31.6%), UDH(9.4%)(χ2=13.442,22.272, 52.789, P<0.01). There was no significance in DCIS and ADH(χ2=2.1878, P=0.139).(2) The MVD based on CD105 expression had statistical significance in IDC-NST(31.691±8.621), DCIS(27.633±6.879),ADH(21.436±5.112) and UDH groups(10.038±3.976), MVD adjacent to the lesion was significantly higher than those within the lesion in every group(P < 0.01 for each group).(3) There was positive correlation in over-expression of HIF-1α, CD105-MVD with the level of ER or PR, histological grade, lymph node metastasis and distant metastasis of IDC-NST(P<0.01), and the expressions of the two proteins were not related with age andsize of tumor(P>0.05).(4) There was significance of the expression of CD105-MVD between the groups of HIF-1α positive and negative(P<0.01). Conclusions The abnormal expression of HIF-1α may be as an early event in the development of breast tumor. The over-expression of HIF-1α, CD105-MVD might be important biological markers for invasion, lymph node metastasis and distant metastasis of IDC-NST. The combined detection of HIF-1α and CD105 may be as the predictors for prognosis and targeting treatment of breast carcinoma.
