S100B蛋白和APACHEⅡ评分对心肺复苏后患者脑损伤的评估

【摘要】 目的 研究心肺复苏(CPR)患者早期血浆S100B蛋白浓度和APACHEⅡ评分对脑损伤预后的评估价值。方法 根据入选的52例患者脑功能预后情况,分为预后不良组28例和预后良好组24例,比较两组患者CPR成功后24 h血浆S100B蛋白浓度和APACHEⅡ评分,绘制ROC曲线,进行统计学分析。结果 预后不良组血浆S100B蛋白浓度和APACHEⅡ评分均高于预后良好组(P<0.01);S100B蛋白预测CPR后患者继发脑功能不良的曲线下面积0.843,APACHEⅡ评分曲线下面积0.821;CPR术后24 h患者血浆S100B蛋白浓度和APACHEⅡ评分具有正相关性(r=0.67,P<0.01)。结论 CPR成功后24 h患者血浆S100B蛋白浓度增高、APACHEⅡ评分增大,预示脑功能预后不良;二者可以用来早期评估CPR后患者脑损伤预后。

【Abstract】 Objective To evaluate the effects of plasma S100B protein and APACHE Ⅱ score on brain damage at the early stage after cardiopulmonary resuscitation(CPR).Methods A total of 52patients from August 2011 to August 2013 with cardiac arrest in- hospital or out- of- hospital and successfully restored spontaneous circulation(ROSC),the level of plasma S100B protein was detected24 hours after CPR.At the same time,the clinical data of chronic health evaluation(APACHE) Ⅱ were collected.These patients were divided into unfavorable prognosis group(n = 28) and favorable prognosis group(n = 24) according to cerebral performance category(CPC) 3 months after of CRP,the level of S100B and APACHE Ⅱ scores were compared between two groups.Data were analyzed and receiver operating characteristic(ROC) curves were made.Results Plasma S100B protein and APACHE Ⅱscores of unfavorable outcome group were obviously higher than those in the favorable outcome group(P<0.01).Plasma S100B protein had positive correlation with APACHE II scores(r = 0.67,P <0.01).ROC curves for unfavorable prognosis of patients revealed an area under the curve of 0.843 for S100B,and 0.821 for APACHE Ⅱ.Conclusion The level of plasma S100B protein and APACHE Ⅱscore can be used to evaluate brain damage at the early stage after CPR.
