【摘要】 目的 了解过渡ICU在急诊危重患者管理中的价值。方法 2012-01~2012-06采用EICU患者病情稳定后转入普通病房模式和2012-07~2012-12采用EICU患者病情稳定后转入过渡ICU(progressive care unit,PCU)模式,比较分析两种模式下患者重返ICU率、死亡率和EICU平均住院日。结果 ①EICU患者转入普通病房组重返ICU率达25%,转入PCU组重返ICU率为5%,普通病房组重返ICU率明显高于PCU组,两组比较差异有统计学意义;②转入普通病房组患者住院死亡率明显高于转入PCU组(15.6%vs 2.5%),两组比较差异有统计学意义。③两组在不同模式下,EICU的平均住院日分别为(10.53±3.60)d vs(9.23±3.73)d,两组比较差异无统计学意义。结论 PCU可以降低危重患者重返ICU率及住院病死率,改善危重患者预后,是值得推广的急诊危重患者管理模式。更多还原

【Abstract】 Objective To investigate the value of Progressive Care Unit(PCU) for severe patients in emergency department.Methods We conducted an analysis of all patients admitted to the Emergent ICU(EICU) between January 2012 and December 2012.Before July all patients discharged from ICU were transferred to general ward and after July patients to PCU.The readmission rate,mortality in hospital and average length of stay were analyzed.Results ①The readmission rate of general ward group was 25%.The readmission rate of PCU group was 5%.The readmission rate of the patients who were transferred to PCU was significantly lower than that of transferred to general ward.②In-hospital mortality was significantly lower in patients who admitted to the PCU(15.6%versus 2.5%;P <0.001) than that in patients who transferred to general ward.③ICU length of stay during the PCU or general ward models were(10.53 ±3.60) d vs(9.23 ±3.73) d and showed no significant difference.Conclusion For the patients who discharged from EICU,PCU model may decrease the readmission rate and in-hospital mortality and is worth popularizing.更多还原
