【摘要】 目的探讨一站式增强T1高分辨率各向同性容积采集(e-THRIVE)检查对食管胃底曲张静脉成像方法及技术要点。方法回顾性分析83例肝病患者进行肝脏MRI常规平扫及增强检查资料,增强扫描使用e-THRIVE技术进行横轴位容积扫描,对原始图像进行最大信号强度投影处理,观察曲张的食管胃底静脉显示并分析病理血管范围、部位。结果 83例患者中74例患者显示门脉及扩张增粗的食管胃底静脉,获得较理想的图像,5例患者因未能控制好呼吸,食管静脉血管模糊,不能辨认血管为食管黏膜下或食管壁外血管,4例轻度食管静脉曲张患者,食管静脉显影较差。诊断食管胃底静脉曲张79例,其中胃底静脉曲张合并食管静脉曲张27例,单纯胃底静脉曲张19例,单纯食管静脉曲张33例,无异常发现4例。最大信号强度投影图像可显示曲张血管整体效果,横轴位图像可显示曲张血管位于黏膜下或管腔壁内外。结论 MRI一站式增强e-THRIVE技术不但显示肝脏疾病,在适量的造影剂用量、注射速率,门静脉期两次扫描及患者良好屏气配合的情况下能很好显示食管胃底曲张血管。更多还原

【Abstract】 Objective To investigate the imaging technique and clinical application of enhanced T1high resolution isotropic volume examination( e-THRIVE) one-step checking technology in the diagnosis of gastroesophageal varices. Methods Eighty-three patients with liver diseases underwent routine and contrast-enhanced MRI scan. The e-THRIVE axial volume scan was used in contrast-enhanced MRI. The raw data were post-processed with maximum intensity projection( MIP) technique,and the esophageal and gastric varicose veins were observed and the range and location of pathological vessels were analysed. Results Portal vessel and esophageal and gastric vessels were thickened and dilated in 79 cases. Because of breathing uncontrolled,vessel imagings of 5 cases were unclear and could not be judged the locations of vessels. 4cases showed mild esophageal varices. In 79 patients with gastroesophageal varices,27 cases showed gastric varices combined with esophageal varices,19 cases showed gastric varices only,and 33 cases showed esophageal varices. The MIP images showed the overall views of varicose vessels,and the axial images displayed the location of varicose vessels,in submucosa or inside or outside of the lumen. Conclusions eTHRIVE not only can show liver disease,but can show esophageal and gastric varicose vessels with moderate contrast agent,injection rate,the repeated scanning in portal venous phase and patients with good breathhold.更多还原
