【摘要】 目的:探讨自体骨髓干细胞移植治疗新鲜颈中型股骨颈骨折,预防股骨头缺血坏死临床疗效。方法:对44例新鲜颈中型股骨颈骨折患者施行自体骨髓干细胞移植配合采用牢固内固定AO技术,对照组43例单纯采用AO3枚空心加压螺钉内固定,两组进行观察复查MRI影像学检查和髋关节Harris评分。结果:12、24月后预防组和对照组分别进行髋关节Harris评分:预防组分别为67.21±10.82,86.43±11.52;对照组分别为54.72±12.23,74.32±14.43;两组比较差异有统计学意义(P<0.05);术后12月予MRI检测,预防组股骨头坏死3例,对照组10例,两组比较差异有统计学意义(P<0.05);24月后MRI股骨头坏死预防组5例,对照组13例两组比较差异有统计学意义(P<0.05)。结论:新鲜颈中型股骨颈骨折患者施行自体骨髓干细胞移植配合采用牢固内固定AO技术方法简便,安全有效,对新鲜颈中型股骨颈骨折能有效预防和减少股骨头坏死发生率,改善关节功能,患者依从性好,是治疗新鲜颈中型股骨颈骨折的一种新径。更多还原

【Abstract】 Objective:To investigate the preventive effect of autologous bone marrow stem cell transplantation on avascular necrosis of the femoral head in treatment of fresh femoral via-neck type fracture.Methods:44 cases of fresh femoral via-neck type fracture were treated by autologous bone marrow stem cell transplantation combined with AO internal fixation technique.43 cases of the control group are treated with three AO cannulated screw.MRI examination was performed and therapeutic effect was evaluated with Harris score in two groups.Results:12 and 24 months later,Harris scores in prevention group were 67.21±10.82,86.43±11.52 respectively and in control group Harris scores were 54.72±12.23,74.32±14.43 respectively.There was statistical difference between the two groups(P<0.05).12 months after surgery,3 cases of femoral head necrosis was revealed by MRI examination in prevention group while 10 cases in control group,and there was statistical difference between the two groups(P<0.05).24 months after surgery,5 cases of femoral head necrosis was revealed by MRI examination in prevention group while 13 cases in control group,and there was statistical difference between the two groups(P<0.05).Conclusion:Autologous bone marrow stem cell transplantation combined with AO internal fixation is a simple,safe and effective method for preventing avascular necrosis of the femoral head in treatment of fresh femoral via-neck type fracture.It can improve articular function and has good compliance.It’s a new method in treating fresh femoral via-neck type fracture.更多还原
