TGFβ1和CD4+CD25+调节性T细胞表达在系统性红斑狼疮发病中的意义
Expression of transforming growth factorβ1 and CD4+CD25+ regulatory T cells in patients with systemic lupus erythematosus
【摘要】 目的检测系统性红斑狼疮(SLE)患者外周血转化生长因子β1(TGFβ1)和CD4+CD25+调节性T细胞(Treg)的表达,探索其在SLE发病中的作用及临床意义。方法分别采用双抗体夹心酶联免疫吸附试验(ELISA)和二色荧光抗体标记流式细胞仪法,对SLE患者和正常对照组血清TGFβ1和外周血CD4+CD25+调节性T细胞进行检测,并结合临床资料进行分析。结果血清TGFβ1和外周血CD4+CD25+调节性T细胞表达在SLE活动组[(715±414)pg/ml、(4.53±0.85)%]明显低于正常对照组[(1314±129)pg/ml、(8.78±2.87)%]和SLE稳定组[(1423±153)pg/ml、(10.89±1.41)%](均P<0.05),而SLE稳定组和正常对照组之间差异无统计学意义(P>0.05);SLE患者TGFβ1和CD4+CD25+调节性T细胞表达量均与疾病活动度SLEDAI积分呈负相关(r=-0.68,P=0.002;r=-0.78,P=0.000),与其他临床指标如24h尿蛋白量、抗ds-DNA抗体等无相关性。TGFβ1表达与CD4+CD25+调节性T细胞数之间亦未见相关性(r=0.51,P=0.113)。此外,血清TGFβ1表达尚与血小板数量和血红蛋白量成正相关(r=0.71,P=0.001;r=0.44,P=0.042)。结论活动SLE患者血清TGFβ1和外周血CD4+CD25+细胞表达减少,提示两者可能参与SLE发病,并对判断疾病活动度有一定的临床意义。
【Abstract】 Objective To investigate expression and clinical significance of transforming growth factor β1 and CD4+CD25+ regulatory T cells in patients with systemic lupus erythematosus (SLE) to explore their roles in the pathogenesis of lupus. Methods Serum TGFβ1 and percentage of peripheral blood CD4+CD25+ regulatory T cells from SLE patients and normal controls were measured using ELISA or flow cytometric assay, then correlatively analyzed with clinical da ta. Results Serum TGFβ 1 and percentage of CD4+CD(25)+ regulatory T cells were significantly lower in active SLE patients [(715±414)pg/ml and (4.53±0.85)%] than stable SLE patients [(1423±153)pg/ml and (10.89±1.41)%] or normal controls [(1314±129)pg/ml and (8.78±2.87)%] (P < 0.05). Both inversely correlated with SLEDAI (r = -0.68, P = 0.002 and r = -0.78,P = 0.000) in SLE while not with other clinical data such as complements, anti-ds-DNA et al. There was no correlation between TGFβ 1 and CD4+CD25+ regulatory T cells. Furthermore, serum TGFβ 1 correlated with platelet number (r =0.71, P = 0.001) and hemoglobin (r = 0.44, P = 0.042) in SLE. Conclusion TGFβ1 and CD4+CD25+ regulatory T cells may play certain roles in the pathogenesis of SLE and may be useful factors in assessing clinical activity.
