参芎葡萄糖注射液对慢性肺源性心脏病血液流变学及血流动力学指标的影响

【摘要】 目的观察慢性肺源性心脏病患者血液流变学及血流动力学指标的变化以及参芎葡萄糖注射液对其的影响。方法选择慢性肺源性心脏病患者96例,将其随机分为两组:治疗组50例,在常规治疗基础上加参芎葡萄糖注射液100 ml静滴,2次/d,疗程为14 d。对照组46例,给予吸氧、抗感染、解痉平喘祛痰等常规治疗。采用血液流变学测定仪测定血液流变学指标,采用多普勒超声心动图检查心脏排血指数、左室射血分数、每搏量、心排血量等血流动力学指标的改变情况。结果血液流变学指标变化:治疗组和对照组治疗后的红细胞沉降率、纤维蛋白原、全血黏度(高切、低切)、血细胞比容较治疗前均有下降(P<0.05);治疗组下降更明显,两组比较差异有统计学意义(P<0.05)。血流动力学指标变化:治疗组和对照组治疗后反映心脏舒张、收缩功能的心脏排血指数、左室射血分数、每搏量、心排血量均有明显增加,治疗组增加幅度大于对照组,两组比较差异有统计学意义(P<0.05)。结论参芎葡萄糖注射液可以明显改善慢性肺源性心脏病患者的血液黏度,改善微循环,并能增加慢性肺源性心脏病患者的心脏舒张及收缩功能,可以作为慢性肺源性心脏病急性发作期改善血液流变性及血流动力学的常规治疗药物之一。

【Abstract】 Objective To observe the effects of salivae miltiorrhizae liguspyragine hydrochloride and glucose injection on blood rheology and hemodynamic indexes of chronic pulmonary heart disease.Methods Ninety six patients with chronic pulmonary heart disease were selected and randomly divided into treatment group and comparative group.Fifty patients in the treatment group,besides the conventional treatment,were also treated by intravenous drip of salivae miltiorrhizae liguspyragine hydrochloride and glucose injection(100 ml) bid for 14 days.Forty six patients in comparative group received conventional treatment such as inhaling oxygen,being immunized against infection,getting antispasmodic and dispelling phlegm therapy.Hemeorheology instruments were adopted to measure the blood rheology index.In addition, Doppler echocardiography was used to check changes of hemodynamic indexes including cardiac index,left ventricular ejection fraction,stroke volume,and cardiac output.ResultS Changes of blood rheology indexes:After treatment, patients from both groups experienced lower erythrocyte sedimentation rate,fibrinogen,whole blood viscosity(high shear and low shear),and hematocrit.The results of the treatment group were much more evident than those of the comparative group(P<0.05),differences between the two groups were statistically significant(P<0.05).Changes of hemodynamic indexesxardiac systolic and diastolic function indicators as cardiac indexjeft ventricular ejection fraction,stroke volume, and cardiac output improved significantly for both groups and patients in treatment group were observed with greater improvement,differences between the two groups were statistically significant(P<0.05).Conclusion Salivae miltiorrhizae liguspyragine hydrochloride and glucose injection can significantly improve chronic pulmonary heart disease patients’ blood viscosity,microcirculation,cardiac systolic and diastolic function.Therefore it can be used as one of the conventional drugs to improve blood rheology and hemodynamics for acute exacerbation of chronic pulmonary heart disease.
