【摘要】 目的分析中晚期患者化疗前后肿瘤性贫血血红蛋白的变化与疗效的关系。方法回顾性分析我院2010年4月～2012年12月接受化疗的146例中晚期恶性肿瘤患者,对其病种、化疗方案及化疗前后血红蛋白变化进行统计。结果初治前贫血(A组)46例,发生率为31.51%,无贫血组(B组)100例共化疗354个疗程,化疗后1个周期贫血发生率为48.00%,2个周期后贫血发生率为51.00%,3个周期后贫血发生率为56.79%,4个周期后贫血发生率为59.72%。在354个疗程中,轻、中度贫血产生188次,轻度贫血发生率37.77%(71/188),中度贫血发生率62.23%(117/188)。经统计A组轻度贫血与初治后贫血(C组)轻度贫血治疗有效率分别为46.22%和32.39%,差异有统计学意义(P<0.05);A组中度贫血与C组中度贫血治疗有效率分别为30.0%和19.66%,差异有统计学意义(P<0.05)。结论化疗效果同血红蛋白浓度呈正相关,血红蛋白浓度影响化疗效果,治疗前贫血患者病因多与肿瘤相关,化疗后贫血可能存在耐药因素,影响疗效,因此在化疗的同时进行早期干预贫血的治疗,对减少耐药因素具有重要意义。更多还原

【Abstract】 Objective To analyze the relationship between changes of hemoglobin in cancer-related anemia pre and post chemotherapy at the middle and advanced stage and therapeutic effects.Methods 146 cases with malignant tumors at the middle and advanced stage accepted chemotherapy in our hospital from April 2010 to December 2012 were analyzed and the species of diseases,chemotherapy regimens,and hemoglobin levels before and after chemother apy were statistically analyzed.Results There were 46 cases with anemia before chemotherapy(group A),accounted for 31.51%.100 patients with non-anemic(group B).354 courses of treatment were lasted for all participants.After chemotherapy,the incidence of anemia in the first,second,third,and fourth cycle was 48.00%,51.00%,56.79%,and 59.72% respectively.During the courses of treatment,anemia in mild and moderate degrees came up 188 times,and the incidence of mild anemia was 37.77% and in moderate degree was 62.23%.In statistics,the effective rate for mild anemia in group A and group C was 46.22% and 32.29%(P<0.05).The therapeutic effective rate of moderate anemia in group A and group C was 30.00% and 19.66% with a statistical difference(P <0.05).Conclusion The effect of chemotherapy is positively correlated with the concentration of hemoglobin,which indicates the concentration of hemoglobin has some influence on the effect of chemotherapy.Anemia before treatment,the pathogenesis in patients with therapeutic effects is mostly related with tumor,while after treatment,the factor of drug resistance is probable in fluencing on the effect.Therefore,early intervention treatment on anemia is of great significance on reducing drug re sistance on chemotherapy simultaneously.更多还原
