不同糖耐量绝经后女性胰岛β细胞功能与血浆纤溶酶原激活物抑制剂1和组织型纤溶酶原激活物的相关性
Beta cell function in relation to plasminogen activator inhibitor-1 and tissue-plasminogen activator in postmenopausal females with different glucose tolerance
【摘要】 目的探讨不同糖耐量状态下绝经后女性β细胞功能与雌激素、血浆纤溶酶原激活物抑制剂1(PAI-1)及组织型纤溶酶原激活物(t-PA)的关系。方法绝经后妇女123人。糖耐量正常(NGT)38人,糖耐量受损(IGT)51例,2型糖尿病(T2DM)34例。采用ELISA法测定t-PA、PAI-1,放射免疫法测定胰岛素、雌二醇、孕酮、促卵泡刺激素(FSH)、促黄体生成素(LH)。计算稳态模型胰岛素分泌指数(HOMA-B)和胰岛素抵抗指数(HOMA-IR)。结果 T2DM组雌二醇较NGT、IGT组低[(35.46±6.46)比(40.26±1.98)、(37.92±4.95)nmol/L],FSH、LH高于NGT组[(41.66±8.77)比(35.21±7.89)U/L,(32.12±7.76)比(27.46±7.06)U/L,均P<0.05]。T2DM组纤维蛋白原(Fg)、PAI-1高于NGT组,t-PA低于NGT组[(3.97±0.68)比(3.49±0.91)g/L、(49.7±18.9)比(37.2±13.8)μg/L,(10.21±5.21)比(13.98±6.87)μg/L,均P<0.05]。多元逐步线性回归分析显示雌二醇和HOMA-B是血浆PAI-1、t-PA水平的影响因素。结论绝经后女性β细胞功能与纤溶功能低下相关。
【Abstract】 Objective To investigate the relationship ofβcell function with the concentrations of plasminogen activator inhibitor type 1(PAI-1)and tissue-plasminogen activator(t-PA)in postmenopausal females with different glucose tolerance. Methods A total of 123postmenopausal women were divided into normal glucose tolerance(NGT) group(n=38),impaired glucose tolerance(IGT)group(n=51)and type 2diabetic(T2 DM)group(n=34). The plasma concentrations of t-PA and PAI-1were measured by enzyme-linked immunosorbent assay(ELISA). The levels of insulin,estradiol(E2),progesterone(P),follicle stimulating hormone(FSH)and luteinizing hormone(LH)were determined by radioimmunoassay(RIA). Homeostasis model assessment-B(HOMA-B)and homeostasis model assessment of insulin resistance( HOMA-IR) were calculated. Results The level of E2was significantly lower in T2 DM group than in NGT and IGT group[(35.46±6.46)vs(40.26±1.98),(37.92±4.95)nmol/L], but FSH and LH were higher in T2 DM group than in NGT group[FSH(41.66±8.77)vs(35.21±7.89)U/L;LH(32.12±7.76)vs(27.46±7.06)U/L,all P<0.05]. Fibrinogen(Fg)and PAI-1were higher but t-PA was lower in the T2 DM group than in NGT group[(3.97±0.68)vs(3.49±0.91)g/L,(49.7±18.9)vs(37.2±13.8)μg/L,(10.21±5.21)vs(13.98±6.87)μg/L,respectively;all P<0.05]. Multiple linear regression analysis showed that E2and HOMA-B were influencing factors of plasma levels of PAI-1and t-PA in postmenopausal women. Conclusion βcell function is related to the reduced fibrinolytic function in postmenopausal females.
