早产儿脑室周围-脑室内出血血小板参数及凝血功能临床研究
The clinical research on platelet parameter and blood coagulation function in preterm infants with periventricular-intraventricular hemorrhage
【摘要】 目的:研究脑室周围-脑室内出血(PVH-IVH)早产儿血小板参数和凝血功能的变化及其临床意义。方法:对35例PVH-IVH早产儿(PVH-IVH组)入院24 h内行活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)、纤维蛋白原(Fbg)、D-二聚体(D-D)含量及血小板参数检测。选取同期未发生PVH-IVH的早产儿40例作为对照组。结果:与对照组相比,PVH-IVH组APTT、PT、TT、D-D明显增高(P<0.01),血小板计数(PLT)、血小板比积(PCT)和Fbg明显下降(P<0.01);两组平均血小板体积(MPV)和血小板分布宽度(PDW)比较差异无统计学意义(P>0.05)。结论:PVH-IVH早产儿存在凝血功能障碍及血小板参数异常,可能参与了PVH-IVH发病机制,要重视对早产儿凝血状态的监测,应做好早期干预。
【Abstract】 Objective: To explore the changes of the blood coagulation function and platelet parameter in preterm infants with periventricular-intraventricular hemorrhage( PVH-IVH) and its clinical significance. Methods: The levels of thromboplastin time( APTT),prothrombin time( PT),thrombin time( TT),fibrinogen( Fbg),D-dimer( DD) and platelet parameter were measured in 35cases of PVH-IVH in preterm infants( PVH-IVH group) within 24 hours of admission and 40 cases of PVH-IVH had not occurred over the same period in preterm infant for comparison. Results: Compared with the control group,the levels of APTT,PT,TT and D-D were significantly higher( P all < 0. 01) in PVH-IVH group,while the platelet count( PLT),plateletcrit platelets than the product( PCT),and Fbg decreased significantly in PVH-IVH group( P all < 0. 01). The mean platelet volume( MPV) and platelet distribution width( PDW) had no significant difference between the two groups( P all > 0. 05). Conclusion: This study suggests that there are coagulation abnormalities in preterm infants with periventricular-intraventricular hemorrhage. Platelet parameters abnormalities,may be involved in the PVH-IVH pathogenesis,we should attach importance to preterm infants clotting state monitoring,should make early intervention.
