儿童支原体肺炎60例临床分析

【摘要】 目的探讨儿童支原体肺炎(MPP)的临床特点及治疗方法。方法对60例确诊为支原体肺炎的患儿临床特点进行回顾性分析。结果发病多集中在学龄儿童,7~12岁占65.00%。发热47例(78.33%),咳嗽54例(90.00%),早期肺部体征少。胸部X线检查表现多样性。外周血白细胞大多正常(65.00%),但血沉(68.33%)和C反应蛋白(55.00%)大多轻度升高。21例(35.00%)有肺外合并症。用颗粒凝集法检测血清MP-IgM阳性率高(93.33%)。用荧光探针定量PCR检测咽拭子和支气管肺胞灌洗液MP-DNA,亦有较高的阳性率。所有病例用红霉素、阿奇霉素治疗效果良好,对肺部X线呈大叶性实变影,尤其合并肺不张者,可进行经纤维支气管镜支气管肺泡灌洗术。结论支原体肺炎好发学龄儿童,其肺外并发症常见,颗粒凝集法检测血清Mp-IgM阳性率高,利于早期诊断。红霉素、阿奇霉素治疗效果良好,支气管肺泡灌洗术能促进呼吸道症状的改善及体征恢复,提高治疗的有效率。

【Abstract】 Objective To investigate the clinical characteristics and treatment of mycoplasma pneumonia in children.Methods The clinical date of 60 cases of children with MPP were rolled in the retrospective study.Results The majority cases were school-age children,7-12 years accounted for 65.00%.There were 47 cases with fever(78.33%),54 cases with cough(90.00%).There were less clinical signs with lung early in the diseases.There were a variety of manifestations of the chest X-ray radiography.The majority cases had normal total WBC count(65.00%),but mild rised in the erythrocyte sedimentation rate(ESR)(68.33%) and C-reactive protein(55.00%).There were 21 cases(35.00%) with extrapulmonary complications.There were higher positive rate in using particle agglutination method to detect serum MP-IgM(93.33%).Using fluorescent probe quantitative PCR detection swabs and harsh lavage fluid MP-DNA,also had higher positive rate.Erythromycin or azithromycin is an effective agent to treat MPP.To chest X-ray radiography is lobar consolidation images,especially with pulmonary atelectasis,can be conducted via fiberoptic bronchoscopy bronchoalveolar lavage technique.Conclusions The majority cases of MPP are school-age children,and are common in the extrapulmonary complications.There has higher positive rate in particle agglutination method to detect serum Mp –IgM,which Is helpful to early diagnosis.Erythromycin or azithromycin is an effective agent to treat MPP.Bronchoalveolar lavage technique can promote the improvement of respiratory symptoms and the recovery of the clinical signs and improve the treatment efficiency.
