不同时间服用厄贝沙坦氢氯噻嗪治疗原发性高血压的效果评价

【摘要】 目的探讨不同时间服用厄贝沙坦氢氯噻嗪治疗原发性高血压的临床疗效。方法将78例非杓型原发性高血压患者按随机数字表法分为晨起服药组和夜间服药组各39例,均给予口服厄贝沙坦氢氯噻嗪(150 mg+12.5 mg),1次/天,共治疗8周,观察治疗前后诊室血压及动态血压的变化。结果两组的诊室收缩压(SBP)在治疗后均明显降低(P<0.01),而两组治疗后差异无统计学意义(P>0.05);动态血压结果显示,两组治疗后平均SBP、白天SBP和夜间SBP均有降低(P<0.01),但夜间服药组的夜间SBP下降更明显(P<0.01)。观察期结束后,两组患者血压昼夜节律均有不同程度的改变,差异有统计学意义(P<0.01)。结论厄贝沙坦氢氯噻嗪能有效地降低血压,夜间服药可以更有效地降低夜间的高负荷血压,提高夜间血压达标率,改善非杓型血压昼夜节律向杓状血压改变。

【Abstract】 Objective To investigate the clinical effects of irbesartan/hydrochlorothiazide in treatment of primary hypertension patients. Methods Seventy- eight patients with primary hypertension were randomly divided into the morning medication group( 39cases) and night medication group( 39 cases). All patients were treated with irbesartan / hydrochlorothiazide( 150 mg + 12. 5 mg),1times / day for a total of eight weeks. Clinic blood pressure and dynamic changes in blood pressure were compared before and after treatment. Results Both programs decreased SBP at consulting room,and no difference was found between the two types of treatment. ABPM showed that both programs effectively decreased night SBP,while evening program reduced night SBP more pronouncedly. Circadian rhythm transformed from non- dipper to dipper was found in 17. 95% in the morning program versus 51. 28% in the evening program( P< 0. 01). Conclusions For patients with primary hypertension,irbesartan and hydrochlorothiazide all can effectively reduce blood pressure. However,the night medication can be more effectively to reduce high blood pressure at night,increase the compliance rate of blood pressure at night,and improve the bucket type of blood pressure circadian rhythm to arytenoids blood pressure.
