芬太尼或瑞芬太尼联合丙泊酚用于老年人无痛胃镜检查的麻醉效果比较

【摘要】 目的比较芬太尼、瑞芬太尼联合丙泊酚用于老年人无痛胃镜检查的麻醉效果及其对呼吸、循环功能的影响。方法将115例拟行无痛胃镜检查的老年患者随机分为芬太尼联合丙泊酚组(A组)57例和瑞芬太尼联合丙泊酚组(B组)58例,比较两组患者检查前后的心率(HR)、血氧饱和度(SpO2)、收缩压(SBP)与舒张压(DBP)等指标变化,以及观察两组患者检查中及检查后有无不良反应。结果两组均能达到满意的麻醉效果,但与A组比较,B组丙泊酚总用量较少,对患者呼吸、循环功能的抑制作用较小,苏醒快,检查中、检查后出现的不良反应较少(P<0.05)。结论采用瑞芬太尼复合丙泊酚麻醉用于老年人无痛胃镜检查优于芬太尼复合丙泊酚,既有良好的麻醉效果,又有很好的安全性,是较为理想的麻醉方法。

【Abstract】 Objective To compare the anesthesia effects of fentanyl or remifentanil combined with propofol for elderly painless gastroscopy,and to observe their influence on respiratory and circulatory function.Methods A hundred and fifteen patients undergoing painless gastroscopy were randomly divided into fentanyl combined with propofol(group A,n=57) and remifentanil combined with propofol(group B,n=58).The changes of HR,SpO2,SBP and DBP before and after examination were compared between the two groups.The adverse reactions during and after examination were observed in the two groups.Results Satisfactory anesthesia effects were obtained in the two groups.But comparing the group B with the group A,the total dosage of propofol was smaller,the inhibitory action on patient’s breathing and circulation was lesser with rapid recovery and less adverse reactions during and after examination(P < 0.05).Conclusion Remifentanil combined with propofol is superior to fentanil combined with propofol in elderly painless gastroscopy,which has good anesthesia effect and better safety,and is an ideal anesthesia method.
