【摘要】 目的探讨伴有自杀意念的酒精依赖患者的认知功能状况。方法选择2011年10月至2012年12月在天津市安宁医院住院或门诊就诊的55例伴自杀意念的酒精依赖患者和同期68例不伴自杀意念的酒精依赖患者作为研究对象,比较两组患者的一般资料,并对两组患者的汉密尔顿抑郁量表17(HAMD-17)、汉密尔顿焦虑量表14(HAMA-14)及重复性成套神经心理状态测验(RBANS)评分进行比较。结果①伴或不伴有杀意念酒精的依赖患者间年龄、教育程度、HAMD-17评分及HAMA-14评分比较差异无统计学意义(P>0.05),伴有自杀意念者与不伴有自杀意念者的婚姻状况不同,比较差异有统计学意义(χ2=0.013,P<0.05);②伴自杀意念酒精依赖患者患者视觉广度、注意因子、延时记忆因子分值及RBANS总分均显著低于不伴自杀意念酒精依赖患者组(t=3.567、4.903、6.007、3.103,P<0.05);③Logistic回归分析结果显示,视觉广度、注意因子是酒精依赖患者自杀意念的危险因素。结论伴自杀意念酒精依赖患者存在注意等认知功能损害,视觉广度、注意因子分值低的酒精依赖患者存在自杀意念的危险性可能更高,对有自杀意念的患者进行早期干预,对于患者的康复具有重要意义。更多还原

【Abstract】 Objective To evaluate the cognitive function of the alcohol dependence patients with suicidal ideations. Methods During Oct 2011 to Dec 2012 55 alcohol dependence patients with suicidal ideation and 68 alcohol dependence patients without suicidal ideation from Anning Hospital were recruited in the study. The RBANS questionnaires,HAMD-17,HAMA-14 scales were used to evaluate the cognitive function and the depressive,anxiety state,and the data was compared. Results ① There were no significant difference in age,education level,HAMD-17 and HAMA-14 total scores between the two groups(P > 0. 05),the bachelordom rate of the patient with suicide ideation was higher than the patient without suicide ideation(χ2= 0. 013,P < 0. 05). ②The RBANS total score and the score of visual spatiality and attention delayed memory factor and RBANS were lower in patients with suicide ideation than patients without(t = 3. 567,4. 903,6. 007,3. 103,P < 0. 05). ③ Logistic regression analysis indicated that the lower scores of visual spatiality and attention were the risk factors of suicide ideation in patients with alcohol dependence. Conclusion Cognitive deficit may exist in alcohol dependence patients with suicide ideation,the lower scores of visual spatiality and attention may be the risk factors of suicide ideation,and early intervention is better for the patients with great significance for the rehabilitation.更多还原
