瞬时波强技术评价2型糖尿病早期患者的颈动脉弹性改变
Assessement of Carotid Artery Elasticity in Patients with Early Type 2 Diabetes Mellitus by Using Wave Intensity
【摘要】 目的应用瞬时波强(WI)技术检测2型糖尿病早期患者的颈动脉弹性功能改变,探讨其临床应用价值。方法选择左心室射血分数(EF)>50%的单纯2型糖尿病组(T2DM)21例和糖尿病合并高血压病组(T2DM+EH)25例,正常对照组25例,行颈动脉WI检测,对比分析3组受试者的瞬时减速度波强(W2)、负向波面积(NA)及硬化参数(β)、脉搏波传导速度(PWVβ)、压力应变弹性系数(Ep)、顺应性(AC)。结果 T2DM组与对照组比较,W2增高(P<0.01),β、PWVβ增高(P<0.05),Ep、NA呈增高趋势、AC呈降低趋势,但三者差异均无统计学意义(P>0.05)。T2DM+EH组与对照组比较,W2增高(P<0.05),NA增高(P<0.05),Ep、β、PWVβ明显增高(P<0.01),AC减少(P<0.05)。T2DM+EH组与T2DM组比较,NA、Ep、β、PWVβ呈增高趋势,W2、AC呈降低趋势,但差异均无统计学意义(P>0.05)。结论瞬时波强技术能无创、敏感地检测颈动脉弹性改变,反映动脉硬化的程度,具有重要的临床应用价值。
【Abstract】 Objective To investigate the clinical value of wave intensity(WI) in evaluation elasticity of carotid artery in patients with early type 2 diabetes mellitus and combined with hypertension.Methods Twenty-one patients with early type 2 diabetes mellitus(T2DM) and 25 T2DM patients combined with essential hypertension group(T2DM+EH) were enrolled in this study,and 25 healthy volunteers were taken as control group.Each patient underwent WI exam.The indexes including deceleration wave intensity(W2),negro area of WI NA) and carotid artery elasticity(Ep,β,PWVβ,AC) of right common carotid artery(CCA) in three groups were measured and compared.Results W2 in T2DM group increased more obviously than that in the control group(P<0.01).β and PWVβ in T2DM group were higher than in the control group(P<0.05).Compared with the control group,Ep,β and PWVβ in T2DM+EH group were more obviously higher(P<0.01),while W2 and NA increased significantly(P<0.05),and AC reduced significantly(P<0.05).NA,Ep,β and PWVβ in T2DM+EH group increased than those in T2DM group(P<0.05),while W2 and AC in T2DM+EH group reduced than those in T2DM group,but for all no statistically significant difference was found between the two groups.Conclusion WI can noninvasively and sensitively detect the changes of carotid artery elasticity,which reflected the degree of atherosclerosis in patients with type 2 diabetes mellitus and T2DM combined with EH,and has important clinical value.
