多层螺旋CT在先天性心脏病中的诊断价值
Diagnostic Value of Multi-slice Spiral CT for Congenital Heart Diseases
【摘要】 目的探讨多层螺旋CT(MSCT)在CHD(先天性心脏病)诊断中的优势和价值。方法分析45例经手术确诊的儿童CHD患者MSCT与UCG(超声心动图)检查资料,评价两种检查方法的诊断准确率。结果心内畸形:45例CHD中,手术发现心内畸形51处,MSCT发现45处,误诊1处,漏诊5处;UCG发现49处,误诊1处,漏诊1处。心外畸形:45例CHD中,手术发现心外畸形76处,MSCT发现68处,误诊2处,漏诊6处;UCG发现59处,误诊4处,漏诊13处。其他畸形:MSCT同时发现气管性支气管3例,UCG观察到二尖瓣反流4例。MSCT与UCG诊断准确率比较:心内畸形中,UCG诊断准确率高于MSCT(96.1%vs 88.2%),但无统计学差异(P=0.219);心外畸形中,MSCT诊断准确率高于UCG(89.5%vs 77.6%),有统计学差异(P=0.035)。结论 MSCT显示心外畸形比UCG有更大的优势,在儿童CHD的诊断、随访中起着重要的补充作用。
【Abstract】 Objective To investigate the diagnostic value of multi-slice spiral CT(MSCT) for congenital heart diseases(CHD).Methods A total of 45 cases of CHD confirmed by surgery were collected in the study.All patients had undergone MSCT and ultrasonic cardiogram(UCG) examinations.The diagnostic accuracy of these two methods was evaluated.Results Intra-ardiac malformation: there were 51 lesions of intra-cardiac malformation displayed by surgery in 45 cases of CHD,and 45 of them were detected by MSCT,of which 1 was misdiagnosed and 5 were missed diagnosis.49 of them were detected by UCG,of which only1 was misdiagnosed and 1 was missed diagnosis.Extra-cardiac malformation: in 45 cases of CHD,there were 76 lesions of extracardiac malformation displayed by surgery,and 68 of them were detected by MSCT,2 was misdiagnosed and 6 were missed diagnosis.59 of them were detected by UCG,of which 4 were misdiagnosed and 13 were missed diagnosis.Other malformations: there were 3 cases of tracheal bronchus demonstrated by MSCT,and 4 cases of mitral regurgitation by UCG.Comparing of diagnostic accuracy between MSCT and UCG: in intra-cardiac malformation,the diagnostic accuracy of UCG was higher than MSCT(96.1% vs 88.2%),but it had no significant difference(P=0.219).In extra-cardiac malformation,the diagnostic accuracy of MSCT was higher than UCG(89.5% vs 77.6%),and it had significant difference(P=0.035).Conclusion MSCT is superior to UCG on extra-cardiac malformations,and it has an important role in diagnosis and follow up of CHD.
