双黄连注射剂导致类过敏反应的特点

【摘要】 目的研究双黄连注射剂类过敏反应的特点。方法采用大鼠、犬和小鼠3种动物模型。雌性BN大鼠单次iv注射双黄连注射剂187.5,375(相当于临床剂量)和750mg·kg-1;雌性比格犬单次iv注射双黄连注射剂56.4,112.8(相当于临床剂量)和225.6mg·kg-1,用生理信号记录仪全程记录血压变化,并在给药前和给药后采集动脉血制备血清,分别用ELISA测定组胺、IgE和补体SC5b-9,C4b和Bb水平。给雌性小鼠单次尾静脉注射伊文思蓝溶液,15min后单次sc注射双黄连注射剂270,540(相当于临床剂量)和1080mg·kg-1,根据小鼠背部皮肤蓝斑面积和皮肤组织中伊文思蓝浓度,比较血管渗透性强弱,反映类过敏反应发生的严重程度。结果①大鼠实验,双黄连注射剂375和750mg·kg-1组血压明显降低(P<0.01),组胺升高(P<0.01),SC5b-9升高(P<0.05),且剂量越高变化值越明显;血清IgE,补体片段C4d和Bb水平未见明显变化。②比格犬实验,双黄连注射剂56.4,112.8和225.6mg·kg-1组血压明显降低(P<0.01),组胺升高(P<0.01),同时SC5b-9升高(P<0.01),C4d亦明显升高(P<0.01),且剂量越高变化越明显;IgE和Bb水平未见明显变化。③小鼠实验,双黄连注射剂270,540和1080mg·kg-1组小鼠皮肤出现明显的蓝斑浸润,皮肤组织浸提液中伊文思蓝浓度明显升高(P<0.01)。结论双黄连注射剂引起类过敏反应的特点是既可激活皮肤中的肥大细胞,又可激活血液中嗜碱性粒细胞,且具有补体介导的类过敏反应的特征。

【Abstract】 OBJECTIVE To study the characteristics of anaphylactoid reaction induced by Shuang-huanglian injection(SHLI).METHODS Rats,Beagle dogs and mice were used to study the characteris-tics of anaphylactoid reaction.In the rat model,rats were once iv injected SHLI 187.5,375(clinical dose) and 750mg·kg-1,separately.In the Beagle dog model,dogs were once iv injected SHLI 56.4,112.8(clinical dose)and 225.6mg·kg-1.Blood pressure was detected by the physiological signal recorder.Ar-terial blood was collected before and after administration,and the serum was used to detect hematological indicators(histamine,IgE,SC5b-9,etc)using ELISA.According to the changes in blood pressure and hematological indicators,the pseudoallergic characteristics of SHLI were determined.In the mouse model,the mice were iv given singly Evans blue in the tail,and 15min later ip injected SHLI 270,540(clinical dose)and 1080mg·kg-1,separately.According to the concentration of Evans blue under the back skin of mice,the severity of vascular permeability was measured and the anaphylactoid reaction was analyzed.RE-SULTS ①In the rat model,SHLI 375 and 750mg·kg-1 induced the release of histamine(P<0.01)and increase in SC5b-9(P<0.05),while the level of C4d,Bb and IgE remained unchanged.②In the Beagle dog model,SHLI 56.4,112.8and 225.6mg·kg-1 induced a significant decrease in blood pressure(P<0.01),the release of histamine(P<0.01),and the increase in SC5b-9(P<0.01)and C4d(P<0.01),while the level of Bb and IgE remained unchanged.③In the mouse model,there was obvious locus coeruleus in-filtration and significant increase in the concentration of Evans blue(P<0.01).CONCLUSION SHLI can cause anaphylactoid reactions,characterized by the ability to activate not only mast cells in the skin but al-so blood basophils.Additionally,this reaction has the feature of anaphylactoid reactions induced by com-plements.
