血管生成、图像融合法容积重现脑内静脉畸形的优势和不足

【摘要】 目的:探讨螺旋CT(MSCT)血管生成和图像融合法容积重现脑内静脉畸形(IVM)的优势和不足。方法:采用64排螺旋CT进行头部容积扫描,利用"血管生长"及"图像融合"法对25例静脉性血管畸形的图像进行容积再现三维重建,分析脑内静脉性血管畸形CTA的影像表现,评价其在脑内静脉畸形诊断和治疗中的优势和不足。结果:25例脑内静脉畸形患者中,CT增强显示条状或放射状排列非解剖部位的异常血管增强影18例,其中5例显示"水母头"征,诊断准确率为66.7%(18/25),采用血管生成及图像融合法进行容积重现,23例IVM直观地呈现典型的"水母头"征,17例可清晰完整地显示颅内静脉畸形的组成及引流静脉的全程,2例显示非解剖部位的异常条状血管,MSCTA对引流静脉全程的显示率为68%(17/25),诊断准确率为92%(23/25)。结论:MSCT采用血管生成和图像融合法进行脑内血管的容积重现,对脑内静脉畸形的诊断存在一定优势,可作为首选的检查方法。

【Abstract】 Purpose:To explore the value of spiral CT with post-processing method of add vessel and merge views in display of brain venous malformations.Methods:Twenty-five patients with venous vascular malformations were undergone 64-slice spiral CT volume scan.The images were reconstructed by blood vessel growth and image fusion method,and were displayed by volume rendering technique.The features of brain venous vascular malformations on CTA images were analyzed.And the advantage and disadvantage of CTA in diagnosis of venous malformation were evaluated.Results:Of the 25 cases of brain venous malformation patients,enhanced CT imaging showed that 18 cases were with strip-like or radially arranged abnormal vascular enhancement,5 cases were with "caput medusae" sign,the display rate of venous malformation on enhanced CT was 66.7%(18/25);CT angiography with image fusion method showed that 23 cases were with typical "caput medusae" sign,17 cases were with malformation vascular structures and draining veins,2 cases were with abnormal strip-like vascular structure,the display rate of venous malformation on MSCTA was 68%(17 / 25),and the diagnostic accuracy was 92%(23/25).Conclusions:Multi-slice spiral CT angiography with image fusion method for the display of brain vascular structures is with certain advantages in the diagnosis of brain venous malformations,and can be used as a preferred screening method.
