【摘要】 目的评价高龄患者髋关节手术应用椎旁-腰丛-坐骨神经阻滞和硬膜外阻滞的麻醉效果。方法选择髋关节手术的高龄患者30例,年龄69-102岁,ASAⅡ或Ⅲ级。将30例患者分为硬膜外麻醉组(Ⅰ组)和椎旁-腰丛-坐骨神经阻滞组(Ⅱ组),每组15例。观察两组阻滞起效时间、麻醉效果、血压、心率变化和麻醉恢复情况。结果两组患者麻醉效果均满意。Ⅰ组麻醉后血流动力学变化较Ⅱ组明显(P<0.05)。与Ⅰ组比较,Ⅱ组患者切口处感觉消失时间和"4"字试验疼痛消失时间短,恶心呕吐发生率和留置导尿率低(P<0.05)。结论老年患者髋关节手术选择椎旁-腰丛-坐骨神经阻滞或硬膜外麻醉均可获得满意效果。椎旁-腰丛-坐骨阻滞麻醉对患者的生理功能影响小。更多还原

【Abstract】 Objective To evaluate the outcomes of paravertebral-lumbar plexus-sciatic(PVLPS) nerve block and epidural block in the elderly undergoing hip surgery.Methods Thirty patients aged from 69 to 102 years old scheduled for unilateral hip surgery were equally randomized into two groups of A(anesthetized with PVLPS nerve block) and B(anesthetized with epidural anesthesia).The onset time of blockade,anesthetic effect and the changes of blood pressure and heart rate were recorded and compared.Results Anesthesia effect was satisfactory in both groups.Compared to group B,group A had less changes of blood pressure and heart rate,earlier recovery of pain feeling,and less incidence of nausea,vomiting and indwelling time of urethral catheter(P<0.05).Conclusion PVLPS nerve block can provide the aged patients undergoing hip surgery with satisfactory anesthesia without remarkable effect on physiological function.更多还原
