【摘要】 目的分析肥厚型心肌病(HCM)患者心电图肥厚新指标与左心室、左房内径(LAD)及舒张功能的相关性。方法 HCM组患者105例中,单纯肥厚型75例(A组),肥厚梗阻型17例(B组),心尖肥厚型13例(C组);选择正常健康人群65例作为对照组。予以常规12导联心电图及超声心电图检查,比较除极化复极化指标、肥厚指标和心电常规参数。并将肥厚指标在HCM不同亚型之间比较,分析新指标与室间隔厚度(IVS)、LAD与左室舒张功能的相关性。结果 HCM组除极化复极化异常及肥厚指标高于对照组(P<0.05)。不同HCM亚型中,C组传统指标SV1+RV5高于A组(P<0.05);B、C组新指标RV3+SV1高于A组(P<0.05)。传统指标∑QRS与LAD、IVS呈正相关(P<0.01);新指标RV2+SV1,RV3+SV1与IVS呈正相关(P<0.01)。随着左室舒张期末内径(LVDd)和LAD的增加,RV2+SV1,RV3+SV1增加(P<0.01)。结论 HCM患者较常人易发生除极和复极化异常,肥厚新指标RV2+SV1,RV3+SV1不仅与IVS有着一定关联度,更能在一定程度上较好评估患者LAD和左室舒张功能,为临床诊疗提供帮助。更多还原

【Abstract】 Objective To analyze the correlation between ECG hypertrophy new indicators and left ventricular internal diameter,left atrial diameter and diastolic function in 105 patients with hypertrophic cardiomyopathy.Methods Consecutive diagnosed HCM patients(group HCM,105 cases) and 65 normal persons(group C) were enrolled and finished ECG and echocardiography.The basic data of polarization,repolarization indicators,hypertrophy indicators and ECG parameters were compared between two groups.The hypertrophy indicators were compared among the subgroups of HCM.The correlations of the new ECG hypertrophy indicators with left ventricular internal diameter,left atrial diameter,and diastolic function were analyzed.Results Compared with group C,the ECG parameters,clinical abnormalities ratios and hypertrophy indexes were higher in group HCM(P<0.05).The differences in repolarization abnormalities among subgroups were significant(P<0.05).There was a positive correlation between ∑QRS and LAD,IVS(r=0.212,r=0.291,P<0.01).The RV2+SV1 and RV3+SV1 were both correlated with IVS(r=0.236,r=0.289,P<0.01).In group HCM,the RV2+SV1,and RV3+SV1 increased as the LVDd and LAD increased.Conclusion HCM patients are more likely to have the abnormalities of depolarization and repolarization.The new hypertrophy indexes RV2+SV1 and RV3+SV1 not only have certain correlation with inter ventricular septum thickness,but also are useful indicators for evaluating left atrial diameter and left ventricular diastolic function.更多还原
