Halliwick技术对学龄期脑瘫儿童平衡和粗大运动功能的影响

【摘要】 目的:观察Halliwick技术对学龄期脑瘫患儿粗大运动功能和平衡功能的影响。方法:选取在广州市康复实验学校就读的痉挛型脑瘫患儿24例,对照组和实验组各12例。两组在性别、年龄、智力、社会适应行为能力、粗大运动功能和平衡能力上差异无统计学意义,具有可比性。对照组采用常规康复疗法,实验组在常规康复疗法基础上应用Halliwick技术进行游泳训练,实验共4个月。实验前后分别评测两组患儿GMFM-88项和平衡能力,实验组训练结束后测评游泳技能。结果:实验组通过4个月Halliwick技术练习,6名患儿可独立游完50~100米。实验前后实验组GMFM-88总分,平衡睁眼时的X轴速度均值、Y轴速度均值、X轴向移动距离、Y轴向移动距离,平衡闭眼时的X轴速度均值、Y轴速度均值、正方形区域边长均有显著性差异(P<0.05)。实验后比较对照组患儿GMFM-88总分较实验组增加,但无显著性差异(P>0.05),除平衡睁眼时X轴速度均值有显著性差异(P<0.05)外,其他平衡能力测试指标无显著性差异(P>0.05)。实验后两组GMFM-88总分无显著性差异(P>0.05)。结论:Halliwick技术可以指导学龄期脑瘫儿童学习游泳,还有助于改善学龄期脑瘫儿童粗大运动功能和平衡协调能力。在运用Halliwick技术指导学龄期脑瘫儿童进行水中运动疗法的过程中,可着重结合神经发育疗法进行水中体位控制训练。

【Abstract】 Objective To study the effect of Halliwick technique on the gross motor function and balance ability of the preschool children with spastic cerebral palsy. Methods Twenty-four preschool children with spastic cerebral palsy were selected as our subjects. They were divided into the control group(conventional rehabilitation therapy,n=12)and the experimental group(conventional rehabilitation plus swimming training,n =12). The GMFM-88 and balance ability of the subjects in two groups was assessed before and after the treatment,and swimming skills of the experimental group was assessed. Results Independently 50-100m swimming was achieved in half of the experimental group after 4 months training. GMFM-88 scores,the average speed of balance in X axis and Y axis with eyes opened,the moving distance of balance in X axial and Y axial with eyes opened,the average speed of balance in X axis and Y axis with eyes closed,and the side length of square area with eyes closed in experimental group after treatment were significant higher than before treatment(P < 0.05). There was no significant difference in above parameters in control group at the same time(P > 0.05),except the moving distance in X axis with eyes opened(P < 0.05). There was no significant difference in GMFM-88 and balance ability between two groups after treatment(P > 0.05). Conclusion Halliwick technique can help preschool children with cerebral palsy learning swimming,improve their motor function and balance ability effectively.
