屈髋屈膝90度前抽屉试验的临床有效性分析

【摘要】 目的:通过测量对比屈髋屈膝90度前抽屉试验与传统前抽屉试验的胫骨前向移动度,检验前者的有效性和优越性。方法:选取单纯单膝前交叉韧带断裂患者36例为病例组,同年龄段健康志愿者40例为正常对照组,应用KT2000分别进行屈髋屈膝90度前抽屉试验和传统前抽屉试验,在最大拉力下测得双侧胫骨前向移动度,进行相关统计学分析。结果:单纯单膝前交叉韧带断裂患者行屈髋屈膝90度前抽屉试验,双膝胫骨前向移动度差值大于传统前抽屉试验,差异有统计学意义(P<0.001)。结论:屈髋屈膝90度前抽屉试验较传统前抽屉试验获得更大的胫骨前向移动度,更准确反映前交叉韧带断裂导致的前向不稳,检出率更高。

【Abstract】 Objective The effectiveness of a new method using anterior drawer test(ADT)at 90° flexion of hip joint and knee joint for testing knee joint anterior stability was verified.The new method was compared with traditional ADT.Methods 36 patients with diagnosed anterior cruciate ligament(ACL)rupture admitted to the Institute of Sports Medicine of Peking University during September 2009 to April 2010 were selected as case group,who underwent both the new method and traditional ADT.40people with normal ACL were selected as a healthy control group.The effectiveness of this new method was confirmed by comparing the difference between the healthy control group and the case group.Both the traditional ADT and the new method were performed on all subjects by KT2000 and the results were compared by paired t tests.Results The new method showed greater anterior displacements on the injured side of patients with ACL rupture than both healthy side of patients with rupture of ACL and the same side of knee of the healthy control group.The difference of anterior displacement on the two knees was 3.50±1.30 mm in traditional ADT,while 5.50±2.29 mm in the new method(P < 0.001).Conclusion The new anterior drawer test at 90° flexion of hip and knee joints more effectively detects the rupture of anterior cruciate ligament than the traditional one.
