【摘要】 目的探讨不同方式植骨融合术联合后路内固定术治疗胸腰椎爆裂性骨折的疗效观察。方法随机选取我院2009年2月至2012年2月胸腰椎爆裂性骨折患者90例,随机分两组,治疗组50例给予一期后路内固定术及椎体加椎间植骨术,对照组40例行一期后路内固定术和后外侧植骨术,观察两组患者的手术时间、术中出血量、术后伤椎Cobb角、骨性融合率、内固定失效率和Frankel分级。结果治疗组术中出血量和手术时间分别为(357.27±31.67)mL和(102.17±18.61)min,优于对照组,具有统计学差异(P<0.05);术后治疗组伤椎Cobb角(3.48±0.51)度,显著低于对照组;骨性融合率和内固定失效率分别为98.0%和2.0%,均明显优于对照组(P<0.05);治疗组术后Frankel分级明显优于对照组(P<0.05)。结论后路内固定术后联合椎体加椎间植骨术治疗胸腰椎爆裂性骨折的疗效显著,是最优的骨性融合方式,值得临床推广。更多还原

【Abstract】 Objective To assess the effectiveness of different bone graft fusion ways combined with posterior short segment internal fixation in treating thoracolumbar burst fractures.Methods 90 patients of thoracolumbar burst fractures in our hospital from February 2009 to February 2012 were selected and randomly divided into two groups.Patients in the treatment group were treated with posterior short segment internal fixation combined with interbody bone graft fusion,and patients in the control group were given posterolateral bone graft fusion.The operation time,bleeding volume,changes of the Cobb angle,bone fusion rate,internal fixation failure rate and Frankel grade of the fracture vertebral were observed.Results The bleeding volume and operation time in the treatment group was (357.27 ± 31.67) mL and (102.17 ± 18.61) min,significantly lower than the control group (P <0.05).The patients’ Cobb angle in treatment group was lower than that in control group,the bone fusion rate and internal fixation failure rate were 98.0% and2.0% (P <0.05).Frankel grade also had significant differences between the two groups (P <0.05).Conclusions The centrum combined with interbody bone graft fusion in treating thoracolumbar burst fractures has good clinical effect.It is the best way for bone graft fusion,and worth of clinical promotion.更多还原
