雌激素受体与妊娠期肝内胆汁淤积症的相关性研究

【摘要】 目的探讨雌激素受体(ER)与妊娠期肝内胆汁淤积症(ICP)的相关性。方法选择本院住院分娩的70例孕妇作为研究对象,其中ICP患者纳入研究组,正常孕妇纳入对照组;采用放射免疫法对两组孕妇血清雌三醇(E3)和绒毛促性腺激素-β(β-hCG)水平进行测定;收集分娩后胎盘组织,采用免疫组化法测定雌激素受体(ER)、绒毛促性腺激素(HCG)的表达强度。结果研究组E3水平为(18.53±9.73)ng/mL,显著高于对照组的(12.64±5.49)ng/mL(P<0.05);2组β-hCG水平无显著差异(P>0.05);研究组ER阳性表达率为71.88%,显著高于对照组的26.32%(P<0.05);2组HCG阳性表达率经比较差异无统计学意义(P>0.05);Spearman关联性分析结果显示,研究组血清E3与胎盘组织ER表达显著正相关(r=0.604,P<0.05),而血清β-hCG水平与胎盘组织HCG表达无显著相关性(r=0.198,P>0.05)。结论雌激素水平升高以及胎盘中ER表达增强可能在ICP发病中起重要作用。

【Abstract】 Objective To explore the expression of estrogen receptor(ER) and intrahepatic cholestasis of pregnancy(ICP) correlation,to provide the reference for clinical prevention and treatment.Methods Seventy pregnant women who chose to deliver in our hospital were selected as research subjects for the study.The study group included ICP patients,and normal pregnant women were included in the control group.Radioimmunoassay was employed to measure the serum estradiol(E3) and chorionic gonadotropin beta(beta-hCG) levels of the two groups.Placental tissue was collected after delivery,and immunohistochemistry method was used to detect the expression intensity of estrogen receptor(ER) and chorionic gonadotropin(HCG).Results The E3 level of the study group was(18.53±9.73) ng/mL,which was significantly higher than that of the control group(12.64±5.49) ng/mL(P<0.05).while no statistical difference between β-hCG levels in the two groups was found(P>0.05).The positive expression rate of ER in the study group was significantly higher than that in the control group(71.88% vs 26.32%)(P<0.05).The difference of positive rate of HCG expression was not significant between the two groups(P>0.05).Spearman correlation analysis showed that the serum levels of E3 and ER expression in placenta in the study group were positively correlated(r=0.604,P<0.05).The serum-hCG level and expression of HCG in the placenta was not positively correlated(r=0.198,P>0.05).Conclusion Estrogen levels and increased expression of ER in placenta may play an important role in the pathogenesis of ICP.
