MRI肺灌注成像的定量研究及初步临床应用探讨

【摘要】 目的通过比较心功能不全患者与正常对照组肺循环灌注情况,探讨MRI肺循环灌注成像的临床应用价值。方法分别对15例正常对照者和21例心功能不全患者行对比剂增强的MRI灌注成像,采用两个独立样本t检验分别对两组间肺循环相关灌注参数进行统计学比较。结果正常对照组和心功能不全组分别的右室达峰时间为(5.81±2.30)s和(8.91±4.34)s、肺实质达峰时间为(7.12±2.07)s和(12.67±5.29)s、左室达峰时间为(7.75±2.17)s和(14.85±5.94)s、右室-肺实质峰值间隔时间为(3.16±1.79)s和(6.25±3.60)s、肺实质-左室峰值间隔时间为(3.10±1.30)s和(5.34±3.39)s、右室-左室峰值间隔时间为(6.26±1.79)s和(11.59±3.59)s,两组间的各参数的差异均具有统计学意义。结论对比增强MRI灌注成像能够反映不同心功能状态下肺循环参数的情况,具有进一步临床应用研究的价值。

【Abstract】 Objective To assess the feasibility and value of quantitative analysis of pulmonary perfusion using contrast enhancement magnetic resonance imaging(CE-MRI)through the comparison of patients with cardiac dysfunction and controls.Methods 21patients with cardiac dysfunction(Group A)and 15controls without cardiac dysfunction(Group B) underwent pulmonary perfusion CE-MRI.Quantitative perfusion analysis was performed using novel software to assess pulmonary circulation parameters.Two independent samples t-test was used for statistics analysis.Results The time to peak of right ventricle(pTRV),time to pulmonary parenchyma(pTPP),time to left ventricle(pTLV),interval time of pTRV and pTPP(TRV-PP),interval time of pTPP and pTLV(TPP-LV)and interval time of pTRV and pTLV(TRVLV)were respectively(8.91±4.34)s,(12.67±5.29)s,(14.85±5.94)s,(6.25±3.60)s,(5.34±3.39)s and(11.59±3.59)s in group A.And those were(5.81±2.30)s,(7.12±2.07)s,(7.75±2.17)s,(3.16±1.79)s,(3.10±1.30)s and(6.26±1.79)s in group B.There was significant difference between two groups.Conclusion Quantitative pulmonary perfusion using CE-MRI could demonstrate the changes of pulmonary circulation with the difference of cardiac function,which is worth of researching deeply.
