【摘要】 目的探讨重症急性胰腺炎(SAP)经中西医结合治疗与单纯西医治疗前、中、后不同的CT影像特征及症状、体征的变化,评价两种治疗方法的疗效。方法通过回顾性分析2010年1月至2012年12月收治的99例SAP患者的临床资料,按治疗方案不同分为中西医结合治疗组和单纯西医治疗组,比较两组患者治疗前、中、后的CT表现及症状、体征缓解所需时间。结果 99例SAP患者经治疗后,CT影像显示胰腺肿胀、密度及胰周积液均明显改善,但在第3天及第7天复查时,两组CT严重指数记分法(CTSI)评分比较,差异有统计学意义(P<0.05);在治疗第3天及第7天时,中西医结合治疗组症状及体征缓解所需时间明显低于单纯西医治疗组,差异有统计学意义(P<0.05)。本研究中未发生严重不良反应事件。结论中西医结合治疗组在减轻胰腺肿胀、胰周渗出及邻近间隙积液等方面优于单纯西医组,经中西医结合治疗可以较快缓解患者临床症状、体征,缩短治疗时间。更多还原

【Abstract】 Objective To probe the changes of CT image characteristics,symptoms and signs in severe acute pancreatitis(SAP) before,during and after treatment of the combination of traditional Chinese medicine(TCM) and Western medicne(WM) and pure WM,and to evaluate the effects of the two kinds of treatment methods.Methods 99 admitted cases of SAP treated from January 2010 to December 2012 were retrospectively analyzed and divided into the integrated TCM and WM group and the pure WM group according to the therapy schemes.The CT manifestations and time of symptoms and signs alleviation before,during and after treatment were compared between the two groups.Results After treatment in 99 cases of SAP,the CT images revealed that the pancreatic swelling,density and peripancreatic effusion were significantly improved,but the scores comparison of CT severity index(CTSI) assessment in recheck on 3,7 d between the two groups showed statistical difference(P<0.05);the time of symptoms and signs alleviation on 3,7 d of treatment in the integrated TCM and WM group was significantly lower than that in the pure WM group with statistical difference(P<0.05).Conclusion The integrated TCM and WM group is superior to the pure WM group in the aspects of relieving pancreatic swelling,peripancreatic effusion and adjacent interstitial effusion.The combination treatment of TCM and WM can quickly alleviate patients′ clinical symptoms and signs,and shorten the treatment time.更多还原
