【摘要】 目的探讨新辅助化学治疗联合手术治疗食管癌的临床疗效及对患者生活质量的影响。方法 116例食管癌患者分为2组,对照组66例接受单纯手术治疗,观察组50例手术前给予多西紫杉醇联合顺铂的新辅助化学治疗。采用生存质量核心问卷评估患者生存质量,比较2组患者手术切除率、1 a生存率、术后并发症发生率及生存质量。结果观察组患者化学治疗客观缓解率为58.0%,发生骨髓抑制44例(88.0%),消化道反应26例(52.0%)。观察组患者肿瘤根治切除率、总切除率和1 a生存率均显著高于对照组(P<0.05);2组患者术后并发症发生率比较差异无统计学意义(P>0.05)。术后3、6个月及1 a,观察组患者生存质量评分显著高于对照组(P<0.05)。结论新辅助化学治疗联合手术治疗食管癌可显著提高根治切除率,延长患者生存期,改善患者生存质量。更多还原

【Abstract】 Objective To explore the clinical effect of neoadjuvant chemotherapy combined with surgery in the treatment of esophageal cancer and the influence on quality of life of patients.Methods One hundred and sixteen patients with esophageal cancer were divided into two groups,sixty-six patients in control group were treated with surgery alone,while fifty patients in observation group were treated with neoadjuvant chemotherapy consisted of docetaxel and cisplatin before operation.The life quality of patients was evaluated by life quality questionnaire core 30.The resection rate,one-year survival rate,postoperative complications and life quality were compared between the two groups.Results In observation group,the objective remission rate was 58.0%,myelosuppression existed in 44 cases(88.0%),gastrointestinal adverse reactions existed in 26 cases(52.0%).The radical resection rate and total resection rate of tumor,and one-year survival rate in observation group were significantly higher than those in control group(P<0.05).There was no statistical difference in the rate of postoperative complications between the two groups(P>0.05).Three months,six months and one year after operation,the scores of life quality of patients in observation group were significantly higher than those in control group(P<0.05).Conclusion The treatment of neoadjuvant chemotherapy combined with surgery for esophageal cancer can increase the radical resection rate,extend survival time,and improve the life quality of patients.更多还原
