缺血后处理对大鼠肺缺血-再灌注损伤中肺表面活性蛋白A及动脉血气的影响

【摘要】 目的 研究缺血后处理对于肺缺血-再灌注损伤的保护作用。方法 60只雄性SD大鼠随机分为2个模型制作组,每个模型制作组30只随机分为5组:假手术组(S)、缺血-再灌注(1/R)45 min组和120 min组,以及缺血后处理(IPO)45 min组和120 min组。用免疫组化法测定肺组织中肺表面活性蛋白A(SP-A)的表达水平,动脉血行动脉血气分析,测定肺组织的通透性(125Ⅰ标记牛血清白蛋白的漏出量表示),光镜及电镜下观察肺组织的病理形态变化。结果 IPO组与I/R组相比较,SP-A表达水平明显升高(P<0.01),血气分析结果明显改善,肺组织的通透性明显降低(P<0.01),肺组织损伤减轻。结论 缺血后处理可以减轻肺缺血-再灌注损伤,主要表现为肺组织氧合的改善,尤其是减轻组织水肿和减少Ⅱ型肺泡上皮细胞及肺泡表面活性物质的破坏。

【Abstract】 Objective To investigate the effect of ischemia post-conditioning on the rat lung during the ischemia-reperfusion injury by measuring the expression levels of SP-A of the lung tissue and the variation of arterial blood gas.The permeability and the pathological changes of the lung tissue were observed.Methods Sixty male Sprague-Dawley rats were randomized into two model built groups with 30 animals in each group,then each model built group were randomized into five groups with 6 animals in each group:(l)sham operation group(S).(2) I/R 45 mins group(lR45).(3)I/R 120 mins group(IR120).(4)Post-conditioning 45mins group(P45).(5)post-con-ditioning 120 mins group(P120).The expression levels of SP-A of lung tissue were detected by immuno-histochemical stain.Arterial blood gas analysis was conducted.The permeability of lung tissue was determinated(indicating by the leakage of l25I bovine serum albumin).The pathological changes of the lung tissue were observed under light microscopy and electron microscopy.Results In IPO group,the level of SP-A in lung tissue increased markedly compared with I/R group.The permeability of lung tissue decreased significantly,and the pathologic lesion of lung tissues were slight.Conclusion Post-conditioning could reduce lung I/R injury,the lung protection of IPO show by improvement of the oxygenation,relieve of lung tissue edema and inhibition of the destruction of alveolar epithelial cell type Ⅱ and alveolar surfactant especially SP-A.
