【摘要】 目的观察玻璃体切割术治疗外伤性晶状体脱位的临床疗效。方法回顾性分析我院2008年10月至2011年9月收治的外伤性晶状体脱位患者29例(29眼),术前经超声生物显微镜检查显示晶状体悬韧带断裂均超过180°。所有患者均经睫状体平坦部作闭合式玻璃体切割术,切除玻璃体及脱位的晶状体。术后随访3～6月,平均4个月。观察患者术后随访末期视力、眼压、晶状体及并发症情况,并与术前进行比较。结果术后患者最佳矫正视力为0.05～1.0,平均0.45,较术前(平均0.1)明显提高,差异有显著统计学意义(P<0.01)。术后患者眼压为11～37(18.2±1.4)mmHg(1kPa=7.5mmHg),与术前(29.4±8.5)mmHg比较,差异有统计学意义(P<0.05)。术后未见晶状体皮质残留及晶状体源性葡萄膜炎的发生。术后所有患者前房未见玻璃体疝,部分患者出现短暂性角膜水肿,均于术后1～3d消失;所有患者未出现视网膜脱离等严重并发症。结论玻璃体切割术治疗晶状体脱位疗效可靠,术后并发症少。更多还原

【Abstract】 Objective To observe the clinical effect of vitrectomy on traumatic lens dislocation.Methods Retrospective analysis of 29 patients(29 eyes)with traumatic lens dislocation was made in our hospital from October 2008 to September 2011,all patients received the treatment of vitrectomy and lens excision,and the vitreous body and the disarticulated lens were excised.The lens dislocation was ≥180°.The clinical data of with traumatic lens dislocatin.Following up for 3-6 months after operation,and the average was 4 months.The final follow-up visual acuity,intraocular pressure,the lens and the complications were observed and compared with that before operation.Results The best corrected vision was 0.05-1.0,the average was 0.45,it was obviously higher than that before operation(average 0.1),and there was significant difference(P<0.01).The intraocular pressure was 11-37 mmHg(1 kPa=7.5 mmHg),the average was(18.2±1.4)mmHg.Compared with that before operation(29.4±8.5)mmHg,there was significant difference(P<0.05).The cortex resid and uveitis did not appear.Part of retinal edema disappeared 1-3 days after operation.The detachment of retina and the vitreous hernia did not appear in all patients.All patients had no complications of retinal detachment.Conclusion The treatment of vitrectomy on traumatic lens dislocation is reliable,and there are a few complications.更多还原
