利拉鲁肽治疗左心室舒张功能不全的Ⅱ型老年糖尿病患者临床观察
Clinical observation of liraglutide treatment of left ventricular diastolic dysfunction in old patients with type 2 diabetes mellitus.
【摘要】 目的观察利拉鲁肽治疗左心室舒张功能不全的Ⅱ型老年糖尿病患者的临床效果。方法选择体检中心门诊诊断为左心室舒张功能不全的Ⅱ型老年糖尿病患者40例,随机分为治疗组20例,口服二甲双胍及利拉鲁肽1.8mg皮下注射治疗1次/日;对照组20例,单纯使用二甲双胍治疗。用药前及用药26周后分别做超声心动图,检测患者用药后舒张期末室间隔厚度(IVST)、左心室后壁厚度(PWT)、舒张早期最大峰值速度(E峰)、舒张晚期最大峰值速度(A峰)、E/A峰值速度比值及充盈率(PFR)。结果两组患者治疗前后IVST、PWT、E峰、A峰、E/A峰值、PFR均有明显改善,差异有统计学意义(P<0.05)。结论利拉鲁肽除在降糖方面发挥疗效外,对Ⅱ型老年糖尿病的左心室舒张功能不全,有积极的影响,值得进一步研究。
【Abstract】 Objective To observe the clinical effects of liraglutide treatment of left ventricular diastolic dysfunction in elderly patients with type 2diabetes mellitus.Methods 40cases of left ventricular in health examination center in the outpatient diagnosis of diastolic dysfunction in old patients with type 2diabetes mellitus were randomly divided into treatment group with 20cases:Based on informed consent,patients oral metformin and were treated with liraglutide 1.8mg qd subcutaneous injection;control group:20cases patients used metformin simply.Before treatment and after 26weeks of treatment,ultrasound heartbeat graph examination of left ventricular diastolic function in patients with drug detection:detection of posterior interventricular septal thickness(IVST),left ventricular posterior wall thickness(PWT),early diastolic peak velocity(E peak),late diastolic peak velocity(A peak),E/A peak velocity ratio and filling rate(PFR).Result The IVST,PWT,E peak,A peak,A peak,E/PFR of 2groups before and after treatment which compare with control group pressure,has improved significantly,the difference was statistically significant(P<0.05).Conclusion Liraglutide has effect on hypoglycemic,in addition.It has a positive effect on left ventricular diastolic dysfunction in elderly patients with type 2diabetes mellitus,which is worthy of further study.
