【摘要】 目的观察血必净注射液对脓毒症患者血乳酸及APACHEⅡ评分的影响。方法选取60例脓毒症患者,随机分为X组30例和C组30例,C组患者给予常规抗脓毒症治疗,X组患者在常规抗脓毒症治疗的同时加用血必净注射液100 mL静滴,每12 h 1次。观察2组患者基础乳酸值(T0)以及6 h(T1)、12 h(T2)、24 h(T3)血乳酸水平,并计算6 h、12 h及24h的乳酸清除率。比较2组患者入院时及72 h后的APACHEⅡ评分、ICU住院时间和28 d病死率。结果 2组患者T0、T1时点2组血乳酸水平比较均无显著性差异(P均>0.05),X组患者T2、T3时点的血乳酸水平显著低于对照组(P均<0.05),X组患者T1～T3各时点的血乳酸清除率显著高于对照组(P均<0.05);X组患者入院72 h后的APACHEⅡ评分显著低于对照组(P<0.05);2组患者的ICU滞留时间和28 d病死率比较均无显著性差异(P均>0.05)。结论血必净注射液可以降低脓毒症患者血乳酸水平,促进脓毒症患者的乳酸清除,间接改善脓毒症患者的预后。更多还原

【Abstract】 Objective It is to observe the influence of Xuebijing injection on blood lactic acid and APACHEⅡ score of sepsis patients.Methods Sixty cases of adult sepsis patients were divided into Xuebijing therapy group(X group,n = 30) and control group(C group,n = 30).Patients in C group were treated with conventional anti-sepsis cluster therapy.Patients in X group were treated with conventional anti-sepsis cluster plus Xuebijing injection 100 mL once every 12 h.The blood lactic acid level at T0(basis),T1(6 h),T2(12 h),T3(24 h),and lactate clearance rate at T1(6 h),T2(12 h),T3(24 h) of two groups were compared.Basic APACHEⅡscore and 72 h APACHEⅡscore,the ICU stay time and 28 d mortality of two groups were compared too.Results Basic lactic acid in the patients of two groups had no significant difference(P > 0.05),blood lactic acid level of patients in the X group at T2 and T3 were lower than that of the control group,the difference was statistically significant(P < 0.05),the blood lactate clearance rate of X group at T1-T3 was higher than that of the control group,the difference was statistically significant(P < 0.05).The APACHEⅡ score in X group after 72 h were significantly lower than that of C group(P < 0.05).There was no significant difference in ICU stay time and 28 d mortality between the two groups(P >0.05).Conclusion Xuebijing injection can reduce the blood lactic acid levels in patients with sepsis,promote sepsis patients lactate clearance,indirectly improve these patients’ prognosis.更多还原
