结直肠腺瘤患者IGF-Ⅰ、IGF-ⅠR的表达及其与血清IGF-Ⅰ水平的关系
Expressions of IGF-Ⅰ,IGF-ⅠR in Colorectal Adenoma and its Relationship with Serum IGF-ⅠLevel
【摘要】 背景:胰岛素样生长因子(IGF)系统与多种肿瘤的发生密切相关,但其与结直肠腺瘤(CRA)关系的研究目前相对较少。目的:探讨CRA患者IGF-Ⅰ、IGF-Ⅰ受体(IGF-ⅠR)表达及其与血清1GF-Ⅰ水平的关系。方法:选取2011年6月~2012年10月青岛市市立医院50例CRA、30例结直肠癌(CRC)、20例炎性息肉患者。应用免疫组化SP法检测IGF-Ⅰ、IGF-ⅠR表达,放射免疫法测定血清IGF-Ⅰ水平。结果:炎性息肉组、CRA组、CRC组IGF-Ⅰ、IGF-ⅠR表达阳性率呈递增趋势(P<0.05)。CRC组血清IGF-Ⅰ明显高于炎性息肉组(P=0.018)。CRA患者IGF-Ⅰ、IGF-ⅠR表达以及血清IGF-Ⅰ水平与性别、年龄、息肉大小、息肉数量、息肉部位、息肉组织学类型等均无明显关系(P>0.05)。IGF-Ⅰ、IGF-ⅠR阳性表达的CRA患者血清IGF-Ⅰ水平均显著高于IGF-Ⅰ、IGF-ⅠR阴性表达者(P=0.03;P=0.02)。CRA患者IGF-Ⅰ表达与IGF-ⅠR表达呈正相关(r=0.718,P<0.01)。结论:IGF-Ⅰ、IGF-ⅠR表达联合血清IGF-Ⅰ可能成为评估CRA发展和恶变的重要指标。
【Abstract】 Background:Insulin-like growth factor(IGF) system is closely correlated with the pathogenesis of many tumors, however,study on its relationship with colorectal adenoma(CRA) is relatively rare.Aims:To detect the expressions of IGF-Ⅰ,IGF-Ⅰreceptor(IGF-ⅠR) in CRA and its relationship with serum IGF-Ⅰlevel.Methods:Fifty cases of CRA, 30 cases of colorectal cancer(CRC) and 20 cases of inflammatory polyps from June 2011 to October 2012 at Qingdao Municipal Hospital were enrolled.Expressions of IGF-Ⅰand IGF-ⅠR were detected by immunohistochemical SP method, and serum IGF-Ⅰlevel was measured by radioimmunoassay.Results:The positivity rates of IGF-Ⅰand IGF-ⅠR expression in inflammatory polyps,CRA and CRC were increased stepwisely(P<0.05).Serum IGF-Ⅰlevel in CRC group was significantly higher than that in inflammatory polyps group(P =0.018).No correlation was found between expressions of IGF-Ⅰ,IGF-ⅠR and serum IGF-Ⅰlevel and gender,age,size,number,site and histological classification of polyps in CRA patients(P>0.05 ).Serum IGF-Ⅰlevel in CRA patients with positive expressions of IGF-Ⅰ, IGF-ⅠR was significantly higher than that in patients with negative expressions(P = 0.03;P =0.02).There was a positive correlation between expression of IGF- I and expression of IGF- I R in CRA patients(r = 0.718,P<0.01 ). Conclusions;Expressions of IGF-Ⅰ,IGF-ⅠR combined with serum IGF- I level may be used as important indices for assessing the development and malignant transformation of CRA.
