慢性肝病常用血清学指标的回顾性分析

【摘要】 目的通过对慢性肝病患者进行回顾性分析,了解疾病进展至不同阶段时各相关指标的变化情况,从中选取少量指标组合来判断疾病的严重程度。方法运用判别分析方法选取相关指标组建判别方程,然后回代验证判别效果。结果对于慢性肝炎、肝硬化、慢性重型肝炎,PLT、ALP、TBIL、ALB和A/G进入判别方程,通过回代判断,其对三者的正确判别率分别为68.1%、85.0%和70.8%,总体正确率为78.8%;对于慢性轻中度肝炎和慢性重度肝炎,AST、GGT、ALP、TBIL和A/G为最优组合,回代判断显示其正确判别率分别为98.6%和83.3%,总体正确率为95.6%。结论通过PLT、ALP、TBIL、ALB和A/G的把握可初步判断病情严重程度,以便指导治疗;AST、GGT、ALP、TBIL和A/G所拟合的方程有较高的正确性,可以用来区分轻中度肝炎和重度肝炎。

【Abstract】 Objective To explore the changes of common used serological indicators of chronic liver disease as the progression of disease,and choose a small amount of indicators to compose of discriminant function to determine the severity of disease. Methods Discrimant analysis method was used to select the relevant indicators to form discriminant function,then the effect of the discriminant function was validated by back substitution. Results For chronic hepatitis,hepatic cirrhosis and chronic severe hepatitis,PLT,ALP,TBIL,ALB and A / G were the optimal combination. By back substitution of the data,the correct classification rates were 68. 1%,85. 0% and 70. 8%,the overall accuracy was78. 8%; for mild to moderate hepatitis and severe degree hepatitis,the discriminant model composed by AST,GGT,ALP,TBIL and A / G had a good effect. Back substitution analysis showed the correct classification rates of mild to moderate hepatitis and severe degree hepatitis were 98. 6% and 83. 3%,the overall accuracy was 95. 6%. Conclusion The severity of the diseases could be determined initially by grasping the changes of PLT,ALP,TBIL,A / G and ALB.Equation consisited by AST,GGT,ALP,TBIL and A / G has a high accuracy and can be used to distinguish mild to moderate hepatitis and severe degree hepatitis.
