【摘要】 目的比较5种胃癌相关的肿瘤标记物PGC、PTEN、HER-2、CEA及CA72-4在霉菌性胃溃疡和溃疡型胃腺癌组织中的差异表达,探讨其临床病理鉴别诊断价值。方法收集中南大学湘雅二医院病理科2002年3月-2009年12月保存完好的石蜡标本,包括胃镜下胃黏膜活检以及术后胃标本,且已经病理确诊的霉菌性胃溃疡31例,溃疡型胃腺癌35例,以及正常胃黏膜标本10例,采用Elivision免疫组化方法检测PGC、PTEN、HER-2、CEA及CA72-4在各个胃组织标本中的表达。结果在溃疡型胃腺癌中,PGC和PTEN的表达阳性率均低于其在霉菌性胃溃疡中的表达,而HER-2、CEA及CA72-4的表达阳性率则均高于其在霉菌性胃溃疡中的表达,差异有统计学意义(P<0.05);与正常胃黏膜组织相比,霉菌性胃溃疡组织中PGC和PTEN的表达阳性率有所下降,而HER-2、CEA及CA72-4的表达阳性率稍有升高,但差异均无统计学意义(P>0.05)。结论检测PGC、PTEN、HER-2、CEA及CA72-4 5种肿瘤标记物,特别是联合检测对鉴别诊断霉菌性胃溃疡和溃疡型胃腺癌具有较大临床价值。更多还原

【Abstract】 Objective To compare the differential expression of five gastric cancer-related tumor markers including PGC,PTEN,HER-2,CEA and CA72-4 in order to explore the clinical pathological value in differential diagnosis between mycotic gastric ulcer and ulcerated gastric adenocarcinoma. Methods 66 paraffin specimens were collected from post-operation and gastroscopy biopsy including 31 samples of mycotic gastric ulcer and 35 samples of ulcerated gastric adenocarcinoma which have been diagnosed by pathologists from Mar. 2002 to Dec. 2009 in the Second Xiangya Hospital of Central South University. Expression of five common gastric cancer-related tumor markers including PGC,PTEN,HER-2,CEA and CA72-4 were detected by Elivision immunohistochemical technique. At the same time,10 samples of normal gastric mucosa were collected as controls. Results Compared with mycotic gastric ulcer,positive expression rates of PGC and PTEN were significantly lower while HER-2,CEA and CA72-4 were significantly higher in ulcerated gastric adenocarcinoma( P < 0. 05). Though positive expression rates of PGC and PTEN were decreased and positive expression rates of HER-2,CEA and CA72-4 were increased in mycotic gastric ulcer compared with normal gastric mucosa,there was no significant difference between mycotic gastric ulcer and normal gastric mucosa( P > 0. 05). Conclusion Detection of the five gastric-related tumor markers by immunohistochemistry,especially combined detection would benefit the clinical pathological diagnosis differentiating mycotic gastric ulcer from ulcerated gastric adenocarcinoma.更多还原
