左炔诺孕酮宫内缓释系统治疗围绝经期功能性子宫出血的临床观察

【摘要】 目的探讨左炔诺孕酮宫内节育器治疗围绝经期功能性子宫出血的疗效,为寻找有效、合理治疗围绝经期功能性子宫出血提供理论依据并减少子宫切除率。方法对50例围绝经期功能性子宫出血患者放置左炔诺孕酮宫内节育器。结果置环后患者月经量显著减少,治疗前后比较差异有显著学意义;置环后子宫内膜厚度差异有显著性意义;置环3~12月复查,子宫内膜病理显示子宫内膜增生过长改变全部消失,内膜呈现分泌期或间质蜕膜样改变;37例贫血患者在放置左炔诺孕酮宫内节育器后3个月内,贫血均得以纠正。结论左炔诺孕酮宫内缓释系统是治疗绝经过渡期功血有效的治疗方法,对增生过长的子宫内膜也有明显逆转作用,使绝经过渡期女性能顺利进入绝经期,从而避免手术切除子宫。

【Abstract】 Objective: To investigate the clinical efficacy of levonorgestrel-releasing intrauterine system(LNG-IUS) on the treatment of patients with perimenopausal dysfunctional uterine bleeding,in order to provide a new theoretical basis for its clinical benefit in the effective and reasonable treatment of perimenopausal dysfunctional uterine bleeding and to reduce the uterus resection.Methods: 50 patients with perimenopausal dysfunctional uterine bleeding were treated with LNG-IUS.Results: After treatment of LNG-IUS,the menstrual blood volume reduced significantly,with statistical significance.Endometrial thickness reduced significantly after LNG-IUS placement.After 3-12 months treatment of LNG IUS,endometrium pathological examination showed that endometrial hyperplasia had been reversed in all cases.The endometrium exhibited visible secretion and a pseudo-decidual reaction.For 37 anemia patients,their anaemic condition was corrected during 3 months after the insertion.Conclusion: LNG-IUS is an efficient therapy for the menopausal transitional dysfunctional uterine bleeding and can reverse the endometrial hyperplasia effectively which make women during menopausal transition period go into menopause smoothly to avoid surgical removal of the uterus.
